Sample Labels: Samples submitted for laboratory analyses will <br /> clearly identify the particular sample, and include the following: <br /> 1. Site name and number. <br /> 2. Date and time sample was taken. <br /> 3. Initials of collector(s). <br /> 4. Requested Analysis <br /> 5. Preservation <br /> i <br /> Chain-of-Custody: A Chain-of-Custody record will be maintained <br /> for all samples submitted for laboratory analyses, from the time the <br /> j sample was collected to its final disposition. Every transfer of <br /> j custody will be noted and signed for, and a copy of this record will <br /> be kept by each individual who signs. When samples (or groups of <br /> samples) are not under direct control of the individual(s) <br /> responsible for them, they must be stored in a locked and sealed <br /> container. The Chain-of-Custody record will include (at <br /> minimum) the following: <br /> 1. Sample identification number. <br /> € 2. Sample date and time. <br /> €` 3. Name(s) and signature(s) of sampler(s). <br /> € 4. Signature(s) of any individual(s) with control over samples. <br /> 5. Requested analysis. <br /> Custody Seals: Custody Seals demonstrate that a sample container <br /> has not been tampered with, or opened. The individual in <br /> possession of the sample(s) designated for laboratory analysis signs <br /> and dates the seal, affixing it in such a manner that the container <br /> cannot be opened without breaking the seal. <br /> Samples collected for XRF analysis will be labeled with the sample <br /> location and depth, time, and date.
