All entries will <br /> be dated and signed by the individuals making the entries, and <br /> should include (at a minimum) the following information <br /> pertaining to data collection: <br /> 1. Site name and project numbers (TDD & PAN). <br /> 1 2. Name(s) of personnel on-site involved with data collection <br /> activities. <br /> 3. Dates and times of all data collection activities (military <br /> time preferred). <br /> 4. Descriptions of all sampling related activities. <br /> 5. Noteworthy events and discussions. <br /> 1 6.
