LEA Advisory 20 Attachment - Inspection Guidance for SW Landfills Page 13 of 52 <br /> Section 18213(a) requires that every permit must be reviewed every five years after its issuance, most <br /> recent modification, most recent revision, or most recent review. Subsection (b) sets the time frame <br /> for the operator to submit an application for the permit review. This section is in violation if the <br /> operator has not submitted an application for permit review at least 120 days prior to the required <br /> review and the LEA did not complete the review by the due date. <br /> 18222 Report of Disposal Site Information <br /> In order to obtain a solid waste facilities permit, each operator of a disposal site or facility, as <br /> defined in Public Resources Code sections 40121 and 40122, must file with the Enforcement <br /> Agency a Report of Disposal Site Information as required in section 17616 of this title. In order to <br /> maintain the permit, the operator must file amendments to the Report as required in section 17616 <br /> of this title. Such amendments or lack thereof may become the basis for changes in the permit or <br /> for revocation of the permit. A Report of Disposal Site Information shall contain the following: <br /> (a) A descriptive statement of the manner of operation to be conducted at the site. <br /> (b) Information shall be supplied showing the types and relative quantities of wastes to be <br /> received. Specific mention shall be made concerning the receipt of liquid or hazardous wastes. <br /> (c) Indication of the approximate total acreage contained in the site and either the total estimated <br /> capacity in tons indicating in place densities assumed, or the capacity in cubic yards. Also include <br /> a projection of the life expectancy of the site based on current and/or anticipated loadings. <br /> (d) The general location of the proposed disposal site shown on a map of at least the scale size <br /> equivalent to a 1:24,000 USGS topographical quadrangle. Such map shall show points of access <br /> to the site. <br /> (e) A plot plan which delineates the legal boundaries for which clear title is held by the applicant <br /> and/or any parcels which are leased. For all new sites, copies of lease agreements shall be <br /> submitted and substantiation shall be shown that the disposal site owner is cognizant of the <br /> disposal operations and of the responsibilities assigned to the site owner by the standards. <br /> (f) Identification on the plot plan of the specific limits of the existing and planned disposal area(s) <br /> showing relationships to the property boundary lines and adjacent land uses surrounding the site, <br /> distances to the nearest structures shall be identified. <br /> (g) A description of the sequence of development stages of the disposal site operation, giving <br /> tentative implementation schedules for development, usage, site completion and closure. Describe <br /> the extent of change which will occur in areas which will be excavated for the placement of wastes <br /> or for the mining of cover materials. <br /> (h) A map showing the existing topographical contours of the property and proposed final <br /> elevations of the completed disposal site. <br /> (i) Information on the underlying soils, geology, and groundwater occurrence, based on test <br /> borings conducted on the property. <br /> http://www.ciwmb.ca.gov/pe/advisory/20/attach20/20attch l.htm 8/11/98 <br />
