SMIJOAQUIN Environmental Health Department <br /> r 10 <br /> OR I <br /> Facility Name: Facility Address: Date: <br /> ONE STOP MARKET* 1151 W LOUISE AVE, MANTECA December 06,2018 <br /> Item# Remarks <br /> 209 CCR 2716(a-e) Failure to conduct the designated LIST operator visual inspection at least once every 30 days. <br /> The designated operator failed to document the following: <br /> On the 10/19/18 Designated operator(DO)inspection report <br /> -Section 1. missing CERS ID <br /> -Section 111.failed to add a description to all the components marked N/A in section VII through XI. <br /> -Section VI,The owner or operator failed to add a name, a signature and a date within 48 hours of receiving and <br /> reviewing the DO report. <br /> -Section IX,The DO failed to mark Yes or No to the question"Are all sensors in under-dispenser containment <br /> located to detect a leak at the earliest opportunity?" <br /> -Section X,The DO failed to mark N/A instead of No under the question"Has the overfill prevention equipment <br /> inspection been completed within the past 36 months?" <br /> On the 11/19/18 Designated operator inspection report; <br /> -Section Ill.failed to add a description to all the components marked N/A in section VII through XI. <br /> -Section VI,The owner or operator failed to add a name, a signature and a date within 48 hours of receiving and <br /> reviewing the DO report. <br /> -Section IX,The DO failed to mark Yes or No to the question"Are all sensors in under-dispenser containment <br /> located to detect a leak at the earliest opportunity?" <br /> -Section X,The DO failed to mark N/A instead of No under the question"Has the overfill prevention equipment <br /> inspection been completed within the past 36 months?" <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately,and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class 11. <br /> 320 CCR 2635,2637.2,2665,2712 Failure to comply with the installation, inspection, or record keeping of the overfill <br /> Owners or operators of underground storage tanks that do not meet California Code of regulations,title 23 division <br /> 3, chapter 16 section 2635(c)(2) shall test overfill prevention equipment once by October 13th, 2018 and every 36 <br /> months thereafter-,and within 30 days of the date of a repair.The site did not performed the testing by October 13th, <br /> 2018 however the service technician on site was able to conduct the overfill prevention equipment inspection today <br /> (12/06118)while the inspector was on site. Ensure to conduct overfill prevention equipment inspection within the <br /> time frame provided by the regulation and submit test results within 30 days of performing the inspection.
