S0 JOA U 1 Environmental Health Department <br /> U ; i y <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Walgreens#15398 660 W March Ln, Stockton July 16, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> This is a Class II violation. <br /> 104 CCR 66262.34(d)(2)No modified contingency plan. <br /> OBSERVATION:A complete modified contingency plan was not posted at the facility.The facility has a <br /> Consolidated Emergency Response Contingency Plan dated 3/1/2013,found in the facility's hazardous waste <br /> manifest binder,which states the facility has a plumbed eyewash fountain/shower and portable eyewash kits in the 1 <br /> Hour Photo department/location.According to Scott Barnett,the store manager, the contingency plan is outdated. <br /> REGULATION GUIDANCE: The business operator must post the following information next to telephones or in <br /> areas directly involved in the generation and accumulation of hazardous waste: the name and emergency <br /> telephone number of the emergency coordinator, the location of fire extinguishers and spill control material, and the <br /> telephone number of the fire department unless the facility has a direct alarm. <br /> CORRECTIVE ACTION: Immediately post the required information at the facility. Provide a statement to the San <br /> Joaquin County Environmental Health Department that the correction is complete. <br /> This is a Class II violation. <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> OBSERVATION: At the time of inspection, it could not be demonstrated (not necessarily documented)that <br /> employees who handle hazardous waste were properly trained, based on the following observations: <br /> 1. Wal-Zyr Cetirizine Hydrochloride Tablets with expiration date 08/2020 were observed inside a donations box, next <br /> to the hazardous waste storage area,waiting to be sent to the Walgreen Distribution Center(DC)in Woodland, CA. <br /> Scott Barnett,the store manager, stated that the tablets should not have been placed inside the donation box, since <br /> the tablets were due to expire within the next three months. Scott scanned the tablet's barcode and determined that <br /> the tablets are to be managed as toxic hazardous waste. Scott bagged the tablets and placed them inside the toxic <br /> hazardous waste container at the time of inspection. <br /> 2. See violations 102 and 605. <br /> REGULATION GUIDANCE: The business owner must ensure that all employees are thoroughly familiar with proper <br /> waste handling and emergency procedures, relevant to their responsibilities during normal facility operations and <br /> emergencies. <br /> CORRECTIVE ACTION: Immediately train or re-train the employees.
