S0 JOA U 1 Environmental Health Department <br /> u ; i y <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Walgreens#15398 660 W March Ln, Stockton July 16, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 102 CCR 66262.11 Failed to determine if a waste is a hazardous waste. <br /> OBSERVATION: A hazardous waste determination has not been made on the following possible hazardous waste: <br /> 1. Next to the hazardous waste storage area, spillage of what appeared to be two different types of soaps/cleaners <br /> was observed inside a red container storing two bottles of Dove Body Wash, one bottle of Raw Sugar Simply Hand <br /> Wash, and four bath sponges.According to Scott Barnett,the store manager,the spillage has been in the container <br /> since Tuesday, 7/14/20. <br /> 2. Next to the hazardous waste storage area, Bronkaid Ephedrine Sulfate caplets with expiration date 10/2020 were <br /> observed inside a container labeled as"RC Claims Only,""All items must be scanned out Thursday PM for Friday <br /> Pickup,"and "No go backs, unsellable, callins or recalls."According to Scott,the items in the RC Claims container <br /> are going to be sent to back to the Walgreens DC. However, Scott previously indicated that over the counter <br /> pharmaceuticals with an expiration date within the next three months are to be managed as hazardous waste. <br /> 3. The following damaged and/or expired items were observed in open unlabeled containers on a shelf above the <br /> hazardous waste storage area: <br /> -Studio 35 Beauty Daily Clarifying Face Daily Moisturizer expired in 12/2017. <br /> -Five leaky bottles of Nice! Mold& Mildew Remover containing bleach, and a fourth bottle of the Nice! bleach <br /> cleaner bagged with used absorbent. <br /> -At least seven boxes of Bayer aspirin chewable tablets, at least one of which expired in 07/2020. <br /> -Dr. King's AsthmaCare homeopathic medicine expired in 06/2020. <br /> -A damaged box of Bacitraycin ointment expired in 03/2020 and which appeared to have been leaked on by the <br /> Nice! bleach cleaner. <br /> -Five boxes of Optical 20/20 Eye Redness Reliever and Lubricant expired in 01/2019. <br /> -Nine containers of Zim's Max Freeze Lidocain and Menthol Pain Relief Patches, at least three of which were <br /> expired in 03/2019. <br /> -Two damaged boxes of TRP Stye Relief and two damaged boxes of TRP PinkEye Relief, all of which appeared to <br /> have been leaked on by the Nice! bleach cleaner. <br /> According to Scott, these items are"Pull and Quarantine"items,which were removed from the sales floor until the <br /> facility receives a disposition for the items. It was unclear how long ago these items were pulled from the sales floor, <br /> but Scott indicated that the items have been quarantined for a long time. <br /> REGULATION GUIDANCE: Any person who generates a waste shall determine if the waste is a hazardous waste <br /> as specified in Title 22 California Code of Regulations(CCR). There are wastes that are listed as hazardous <br /> wastes. There are wastes that exhibit one or more of the hazardous waste characteristics:toxic, corrosive, reactive <br /> or ignitable. <br /> CORRECTIVE ACTION: Immediately make a hazardous waste determination for each waste , and manage it <br /> according to Title 22 CCR. Use Safety Data Sheets(SDS),waste sampling and test results or other knowledge to <br /> support your hazardous waste determination. Waste testing must be done using methods specified in Title 22 CCR <br /> including sections 66261.20-24. <br /> Submit a statement and supporting documentation with your hazardous waste determination. Demonstrate current <br /> storage and labeling for wastes determined to be hazardous wastes.
