Mr. Nimmo is not listed on the <br /> site's list of designated operators and notification was not provided to the EHD within 30 days of the change. Any <br /> changes shall be reported to the EHD within 30 days. Immediately log into the California Environmental Reporting <br /> System(CERS) at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for <br /> this facility. <br /> This is a minor violation. <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> A designated operator inspection was performed on October 24, 2018 and the next inspection was performed on <br /> November 27, 2018, 34 days later. The designated operator visual inspections shall be conducted within 30 days or <br /> less of the previous inspection. Ensure that all designated operator inspections are performed within the required <br /> time frames. <br /> The designated operator failed to check and note all required items on the visual inspection reports. <br /> -The 10/24/18 inspection report left blank the question regarding UDC sensor locations. <br /> -The 11/27/18 inspection report left blank the tank tightness testing information. <br /> -The 12/13/18 inspection report incorrectly marked overfill prevention testing as 'no.' <br /> -The 10/24/18, 11/27/18, and 12/13/18 inspection reports did not include the 'N' and 'NA' items in Section III. <br /> -Section V of the 1/11/19 inspection report was left blank. <br /> -The 10/24/18, 11/27/18, 12/13/18, and 1/11/19 inspection reports were not signed by the owner/operator. <br /> During the visual inspection, the designated operator shall check that all required testing and maintenance for the <br /> UST system have been completed and shall check all required items on the report. Ensure that designated <br /> operators performing visual inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 301 HSC 25292.1(a) UST system is not operated to prevent unauthorized release, including spills and overfills. <br /> The fuel filters under the dispensers are not located to allow drips and leaks to drain back into the UDC. The <br /> underground storage tank system shall be operated to prevent unauthorized releases. Immediately provide <br /> approved secondary containment for the filters under permit and inspection by the EHD. <br /> This is a Class II violation. <br /> 314 HSC 25290.1, 25290.2, 25291 Failed to have secondary containment or secondary containment not tight. <br /> The fuel filters under the dispensers are not located to allow drips and leaks to drain back into the UDC. Secondary <br /> containment shall be provided for all single-walled UST components.
