SAN" J D�l+ �� I 1411" Environmental Health Department <br /> COUNTY�l <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Dollar Tree#04403 1 125 Lakewood Mall Lodi May 10 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> Based on the types of violations observed,the facility employees are not trained on proper hazardous waste(HW) <br /> handling or the training provided has been inadequate. <br /> Several empty and partially empty chemical containers were found in the HW storage totes.At the time of inspection, <br /> the small amount of used absorbent observed was not enough to account for the amount of missing liquid from the <br /> empty and partially empty containers.When asked what happened to the missing liquid from the empty and partially <br /> empty containers found, David said that the containers could have become empty from either being"store-use"items <br /> to clean the facility, spilling in the store, or spilling in the shipment truck during delivery to the facility.When David <br /> was asked about the empty and partially empty containers listed below individually, he stated he did not know what <br /> specifically happened to the missing liquid from each container. He stated"store-use" items should be marked with a <br /> sticker on the item's barcode, indicating the item was a"store-use" item.At the time of inspection, there were no <br /> items found with a sticker or any other type of marking indicating the items were"store-use." <br /> -An empty 32 fl. oz. container of hydrogen peroxide, in a HW bag, marked STOU, and with a date of 5/8/2019 was <br /> found on a shelf next to the Dollar Tree baskets containing"go-back"items, located at the front of the facility. David <br /> said he had not had a chance to place the item into a HW tote in the back storage area.When David scanned the <br /> barcode on the hydrogen peroxide,the hand-held scanner stated the hydrogen peroxide should be placed in a <br /> State-Regulated, Toxics, OTC Pharmaceuticals, Universal Waste(STOU)HW tote. However, according to the <br /> Products To Be Treated As Hazardous Waste guide posted next to the HW storage area, hydrogen peroxide is to be <br /> placed in an Oxidizers(OX) HW tote. David placed the hydrogen peroxide into an OX HW tote, however he wrote the <br /> store(generator)address incorrectly as 125 N. Ham Ln. Lodi, CA on the HW label. (IMG 6152-3, 6223-5, 6289) <br /> -The items listed below were found in a Dollar Tree basket labeled"Resell Damages,"located on the top shelf above <br /> the Dollar Tree"go-back" item baskets.When asked about the items listed below individually, David did not know who <br /> placed the items in the basket, nor why.According to David, items placed in the"Resell Damages"basket are <br /> processed daily(IMG 6154): <br /> o A HW bag containing two broken deodorant sticks. David said the deodorant could not be resold and should be <br /> processed as HW. David scanned an Arm &Hammer Ultra Max deodorant and a Beauty&Skincare Power Stick <br /> deodorant, both of which were indicated by the handheld scanner to be placed in a STOU HW tote. David marked the <br /> broken deodorant sticks with STOU and today's date. (IMG 6155, 6188-91) <br /> o A HW bag containing what appeared to be paper"Bee Yourself'cards. David marked the cards with STOU and <br /> today's date. (IMG 6155-6, 6191) <br /> o A partially empty 4 fl. oz. container of Assured Regular Strength Stomach Relief Bismuth subsalicylate 525 mg. <br /> The seal on the container was broken. David said the medicine could not be resold and should be processed as HW. <br /> David placed the medicine in a HW bag and marked it with STOU and today's date. (IMG 6157-9, 6191) <br /> o A leaky partially empty 6.5 oz. container of Angel Of Mine Baby Oil Gel. David said the gel could not be resold <br /> and should be processed as HW.The handheld scanner stated the oil gel is not hazardous. However, according to the <br /> Products To Be Treated As Hazardous Waste guide(Guide)on site, Baby Oil is classified as STOU HW. David <br /> placed the oil gel into a HW bag and marked it with STOU and today's date. (IMG 6160-1, 6184, 6191) <br /> o A.25 oz. container of Lip Medex Blistex External Analgesic/Lip Protectant,which appeared to be used. David <br /> said the lip protectant could not be resold and should be processed as HW.The handheld scanner stated the lip <br /> protectant should be placed in a STOU HW tote. David placed the lip protectant into a HW and marked it with STOU <br /> and today's date. (IMG 6181, 6191) <br /> o A container of Nature's Measure Energy Dietary Supplement 21 tablets.
