V\^ANA UI^�A U I N Environmental Health Department <br /> COUNTY— <br /> Small <br /> O IUN tTY <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Dollar Tree#02983 2691 N Tracy Blvd, Tracy May 28. 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS 1,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> Based on the types of violations observed,the facility employees are not trained on proper hazardous waste(HW) <br /> handling or the training provided has been inadequate. Ryan Martinez,Assistant Store Manager, stated he only <br /> processes and handles non-hazardous items in the store, and only the Store Manager, Mary Davis, and the <br /> Merchandise Manager process and handle hazardous items in the store. Mary Davis and the Merchandise Manager <br /> were not present at the time of inspection. <br /> 1.A bottle of Nature's Measure Women's Daily Vitamins was found in a bag labeled as HW, marked with the acronym <br /> STOU and a date of 4/10/19.The bag was also marked with Mary Davis's, Store Manager, initials, according to Ryan <br /> Martinez,Assistant Store Manager.The acronym STOU indicated the item should be placed in a State-Regulated, <br /> Toxics, OTC Pharmaceuticals, Universal Waste HW tote, according to Ryan.The date marked on the bag indicated <br /> the date the vitamins were processed and determined to be placed in the STOU HW tote, according to Ryan.The bag <br /> was found inside an unlabeled tote, located on a shelf beneath the store's baskets containing"go-back"and damaged <br /> items. (IMG 6730-32) <br /> 2.The following was found in an open cardboard box on the floor beneath the store's baskets containing"go-back"and <br /> damaged items. (IMG 6736-8,6741): <br /> a. A partially-empty open bottle of tacky glue. <br /> b. A bottle of glue leaking out onto the bottom of the cardboard box.The spilled glue appeared to be mixed with <br /> spilled blue paint. <br /> Ryan did not know how long the leakage had been there, but believed the items would ultimately be processed as <br /> HW.When Ryan scanned the glue,the hand-held scanner stated the glue should be placed in an STOU HW tote. <br /> 3.A container of Noise Putty was found with leakage on the outside of the container, and missing its lid.The putty <br /> was found on a shelf near the store's baskets containing"go-back"and damaged items.The hand-held scanner stated <br /> "See guide"for the putty's disposal code. Ryan was unsure which HW tote the item should be placed in, since the <br /> Products To Be Treated As Hazardous Waste guide, posted nearby, did not have putty on its list. Ryan did not know <br /> what to do in order to make an accurate HW determination when the scanner nor the guide provided a determination. <br /> (IMG 6730,6742-7) <br /> 4. Hazardous Waste Accumulation Area:Weekly Inspection logs were found on a clipboard located at the HW <br /> storage area.The most recent inspection was completed on April 15, 2019 by Mary Davis.The log for May 2019 was <br /> blank. (IMG 6751-2) <br /> 5. The following items were found in an STOU HW tote with an accumulation start date of 4/10/19(IMG 6755-73): <br /> a.A partially empty 14 fl oz bottle of Palmolive Essential Clean Original dish soap,without any used absorbent or <br /> other spill-clean-up material to account for the missing liquid. Ryan did not know what happened to the missing liquid. <br /> b.A partially empty 12.6 fl oz bottle of Palmolive Essential Clean Raspberry Coconut dish soap,without any used <br /> absorbent or other spill-clean-up material to account for the missing liquid. Ryan did not know what happened to the <br /> missing liquid. <br /> c. An almost empty 30 fl oz bottle of Home Store Island Fresh Dish Soap,with a small amount of seemingly <br /> unused absorbent.The amount of absorbent observed did not account for the amount of missing liquid.
