The failed components must be repaired or replaced by a properly licensed, trained, and certified <br /> contractor under permit and inspection of the EHD. If the failed component can't be replaced immediately, there is a <br /> possibility that the 87 and diesel UST systems may be red tagged to prevent fuel inputs. <br /> Overfill prevention testing for the ATG probes have not been conducted. The tank forms state the ATG probes and <br /> flappers are being used for overfill prevention. This testing was required to be conducted by October 13, 2018. <br /> immediately schedule this testing and provide 48 hours notification to the EHD. Ensure all future testing is conducted <br /> within required time frames. <br /> The overfill prevention test results for 10/08/18 include incorrect and missing information. <br /> -The results did not include manufacturer inspection procedures and tank charts. <br /> -The results indicate 25 pages were attached. No attachments were included. <br /> -The results incorrectly state vent piping is secondarily contained. <br /> -The results incorrectly mark"restricts flow". Mechanical flappers are shut off valves. <br /> -The tank inside diameter was left blank. <br /> -The inches from bottom of tank equals the percent capacity at which overfill activates. <br /> Test result forms must be fully and accurately completed and submitted to EHD within 30 days of testing. <br /> Immediately update these test results to include all of the required information, maintain a copy of the updated results <br /> on site, and submit a copy to the EHD. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> The monitoring system certification was conducted on 05/07/2019 and was not witnessed by EHD. Looked inside <br /> all sumps and UDCs. Verified sensor locations and piping communication. Verified spill bucket 5 gallon capacity. <br /> The paperwork inspection was conducted later in the day on 05/07/2019. <br /> An inspection checklist was provided to the facility operator on the day of inspection.
