The DO failed to mar YES, NO or N/A under the question"Has the tank tightness testing been completed within required timeframes?"and"Has the line tightness testing been completed within the required timeframes?" -Section IX The DO failed to check if all containment sumps that have had a leak detection alarm since the previous inspection and have not been responded to by a qualified UST service technician. -Section X The DO failed to mar YES, NO or N/A under the question"Has the tank tightness testing been completed within required timeframes?"and"Has the line tightness testing been completed within the required timeframes?" Furthermore the DO failed to check that L2-91 fill fuel alarm on 11/30/18 and 5/15/19, L8-87 STP fuel alarm on 12/17/18 and L5-Diesel AB fill fuel alarm on 3/2/19, and 5/18/19, L6-Red diesel fuel alarm on 1/11/19 were responded to appropriately. During the designated operator inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing inspections at least once every 30 days at this facility and are including all of the required information on the reports.
