12-16-1996 d:20PM FROM P. 5 J James Downing December 12, 1996 Page 3 MTICE OF TERMINATION QE SERVICE AP .ALS Under Section 7-091.4 of the Stockton Health and Sanitation Code, Advanced Metal Plating may file a written request for reconsideration with the Director of Municipal Utilities within ten(10)day of the date of this letter, setting forth in detail the facts supporting a request for reconsideration. If, after the Director's reconsideration of the matter, Advanced Metal Plating is still aggrieved, under Section 7- 091.8 of the Stockton Health and Sanitation Code, you may request, in writing, a hearing before a Hearing Officer. Said hearing shall be scheduled within sixty (60) days of receipt of your written request. A Hearing Notice showing the time and date of said hearing will be mailed to you. Under Section 7-09.9 of the Stockton Health and Sanitation Code, any person dissatisfied with the decision of the Hearing Officer may. within thirty (30) days of the notice of the hearing officer's decision, file a written appeal of such decision with the City Manager. The written appeal shall state with specificity the grounds upon which the appeal is being made. The review by the City Manager or his designee shall consist of a review of the record of the administrative hearing. Within thirty, (30) days thereafter, a notice of the Decision and Order shall be issued. That decision shall be final and binding. Any party still aggrieved by the final Order issued by the City Manager may obtain a review of the Order in the Superior Court by filing in the court a petition for "Writ of Mandate" within thirty (30)days following service of a copy of the Decision and Order of the City Manager. Sanitary Sewer Service shall be immediately terminated upon service of a copy of this notice. MORRIS L.
