Please verify the data, and if it does not match the hazardous <br /> waste treatment activity conducted by your company, make the appropriate corrections (see <br /> enclosed instructions). <br /> After verification and correction (if applicable) of the enclosed information, complete <br /> the certification page. Send two copies and appropriate attachments (if applicable) to the <br /> Department's On-Site Hazardous Waste Treatment Unit in Sacramento, and one copy plus <br /> appropriate attachments (if applicable) to the local regulatory agency by certified mail with <br /> return receipt requested.
