N./ INSTRUCTIONS ON-SITE HAZARDOUS WASTE TREATMENT NOTIFICATION RENEWAL FORM The renewal notification form contains the information your company submitted in its initial Permit-By-Rule(PBR) notification. Verify the data printed on this form, using your original notification form as your reference. Each section of the renewal notification form corresponds to the section and . question numbering of the initial notification form. If the data printed out on the renewal form does not match the hazardous waste treatment activity conducted by your company, draw a line through the incorrect data and make the appropriate corrections in the double space provided above it. Mark the appropriate block on the certification page indicating that a change was made. If your company has changed the location of the treatment units, the number of units treating hazardous waste, or changed treatment procedures, you must amend your notification. This form can be used for that purpose by drawing a line through the data to be changed and adding the appropriate data in the double spaces provided. Mark the appropriate block on the certification page and include the required attachments, such as a new plot plan. However, if you add one or more "new" treatment units, you must use the Unit Specific Form (DTSC 1772B,C or D) depending on the tier the unit is in. Call the On-Site Hazardous Waste Treatment Unit at (916) 323-5871 to request the above-mentioned form. The State Board of Equalization will send you a bill for the notification fee. Do not send your notification fee with this notification. Any checks received by the Department will not constitute payment of your notification fee, and will be returned. In order for your company to remain authorized under PBR, it must have a waste minimization program in effect aimed at reducing the volume, quantity, and toxicity of waste generated. When the authorized corporate officer signs the certification page, he or she also certifies that an appropriate waste minimization program is in place. After verification and correction (if applicable) of the enclosed information, the certification page must be completed and signed by an authorized corporate officer or any other person in the company who has operational control and performs decision-making functions that govern operation of the facility. Indicate whether the data in the renewal form were correct and unaltered, were incorrect and therefore altered, or were amended because of operational changes within your company. Box 806 Sacramento, CA 95812-0806 You must also send one copy plus attachments (if applicable) to the local regulatory agency to which a copy of the initial notification was sent. The renewal notifications must be sent by certified mail with return receipt requested. All three copies of the renewal notification form must have original signatures, and must be in ink, preferably of a color readily discernable as original.
