Financial responsibility documents have not been submitted to the EHD. -The Certification of Financial Responsibility form is dated 5/19/17. -The Letter from the CFO has asset and liability amounts that have not been updated since 2015. Current financial responsibility documents are required to be submitted annually. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, and upload the required documents. This is a repeat violation, Class II. 104 CCR 2711 UST operating permit application for facility and tank information not submitted or current. UST Tank Information form for the 87 tank is not current in CERS. -The 87 tank form incorrectly states the riser piping is fiberglass; the risers on site are all steel. Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. This is a Class II violation. 105 CCR 2632(d)(1), 2634(d) Failed to have an approved UST monitoring plan. The monitoring plan is not current and/or not approved by the EHD. -The monitoring plans for all three tanks incorrectly state monitoring equipment service is performed "quaterly". The site conducts annual monitoring equipment service. -The monitoring plans for all three tanks list the mechanical leak detector manufacturer under both the manufacturer and the model sections. -The monitoring plans for all three tanks incorrectly state the monitoring panel is a Veeder-Root TLS-350 under tank, piping, and UDC monitoring. The monitoring panel on site is a Gilbarco EMC. -The monitoring plans for the 91 and diesel tanks left blank the question regarding UDC sensor failure/disconnection. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a minor violation. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. -The map did not include the locations of piping. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. This is a minor violation.
