Annual monitoring system certification, leak detector testing, and spill container testing was performed on May 15, 2018 and a test report was submitted on July 10, 2018, one month late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a minor violation. 201 CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. Maintenance and monitoring records for the last three years were not found on site. -The work order for the repairs to the overfill prevention flapper for the diesel tank (between 9/19/19 and 1/30/19)was not found on site. Mr. Jesse Tiwana stated the work was done by Elite IV. No permit was found in EHD records for any overfill replacement or repair. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies of missing records to the EHD. This is a minor violation. 204 CCR 2712(1) Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation. 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. Afuel drip was observed on the east piping under dispenser 1/2, indicating a leak in the primary piping. All primary containment for the UST system must be tight. Immediately have a properly licensed, trained, and certified contractor repair or replace the failed component, under permit and inspection of the EHD if necessary. This is a minor violation.
