Environmental Health Department <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />May 03, 2019 <br /> Facility Address: <br /> 3538 Manthey Rd, Stockton <br /> Facility Name: <br /> Dollar Tree #05525 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br />5864). Marlene added absorbent to the leaking item to absorb the free liquid (IMG 5805). <br />- An STOU HW tote marked with accumulation start date 1/12/19 and solid as the physical state contained the <br />following bagged items (IMG 5807): <br /> - A leaky container of Ajax with Bleach Powder Cleanser. When Marlene scanned the item, the hand-scanner stated <br />the item should be placed in an STOU HW tote. However, according to the Products To Be Treated As Hazardous <br />Waste guide found in the HW storage area, Ajax Type Powdered Cleaner with bleach is classified as OX. Also, <br />according to the Dollar Tree Potentially Hazardous Material Management poster posted next to the HW storage area, <br />items containing and or labeled with the word “Bleach” are classified as OX. (IMG 5807-12, 5822) <br />- An STOU HW tote marked with accumulation start date 1/22/19, solid as the physical state, and electronic devices <br />as the only contents in the tote, contained the following bagged items (IMG 5813): <br /> - An open leaky container of Palmolive Essential Clean dish soap. The bag the item was placed in contained free <br />flowing liquid without absorbent. The amount of liquid leaked into the bag did not completely account for the amount of <br />liquid missing from the container. The item’s barcode did not have a sticker or any other indication that it was a <br />“store-use” item. Marlene did not know what happened to the missing liquid. Marlene added absorbent to the leaking <br />item at the time of inspection. (IMG 5814-5) <br /> - An open empty 22.5 fl. oz. container of Daily Defense Body Wash. Used absorbent was not found to account for <br />the missing liquid that was once in the container. The item did not have a sticker on its barcode, nor any other <br />indication that it was a “store-use” item. Marlene did not know what happened to the missing liquid. (IMG 5816-7) <br /> - An empty 64 fl. oz. container of LA’s Totally Awesome Fabric Softener. Used absorbent was not found to account <br />for the missing liquid that was once in the container. The item did not have a sticker on its barcode, nor any other <br />indication that it was a “store-use” item. Marlene did not know what happened to the missing liquid. (IMG 5818) <br /> - An empty container of In-Wash Scent Booster beads. Marlene did not know what happened to the beads that were <br />once in the container. The empty container did not have a sticker on the barcode, nor any other indication that it was a <br />“store-use” item. (IMG 5819-20) <br /> - A container of Ajax with Bleach Powder Cleanser. When Marlene scanned the item, the hand-scanner stated the <br />item should be placed in an STOU HW tote. However, according to the Products To Be Treated As Hazardous Waste <br />guide found in the HW storage area, Ajax Type Powdered Cleaner with bleach is classified as OX. Also, according to <br />the Dollar Tree Potentially Hazardous Material Management poster posted next to the HW storage area, items <br />containing and or labeled with the word “Bleach” are classified as OX. (IMG 5821) <br /> - A container of LA’s Totally Awesome Laundry Pre-Wash Stain Remover was found leaking into the STOU HW tote. <br />Marlene applied absorbent to the free liquid in the bag with the item, and cleaned the inside of the HW tote using <br />paper towels. Marlene placed the used paper towels into the bag with the item. (IMG 5823-7) <br /> - A black toner cartridge, which according to Marlene are ordered and used for the facility’s office printer (IMG 5813). <br />- Two boxes containing Compatible Toner Cartridges were found on the top shelf of the HW storage area. The boxes <br />were labeled as "Used," but were not labeled as hazardous waste, as was one black toner cartridge that was <br />previously found bagged inside an STOU HW tote (IMG 5813, 5828-5832). According to Marlene, the used toner <br />cartridges are picked up by the facility's hazardous waste hauler. <br />-An open bag containing several empty packs of GS Neutral Cleaner Concentrate and one empty pack of the cleaner <br />was found outside of the open bag, inside the mop sink (IMG 5839-40, 5841). A Dollar Tree sign posted above the mop <br />sink stated “Never dispose of hazardous materials in the trash or down the drain” (IMG 5840). Empty packs of GS <br />Neutral Cleaner Concentrate were also found in a CB HW tote (IMG 5795-7).
