Environmental Health Department
Small Quantity Hazardous Waste Generator Inspection Report
Date:
May 03, 2019
Facility Address:
3538 Manthey Rd, Stockton
Facility Name:
Dollar Tree #05525
SUMMARY OF VIOLATIONS
(CLASS I, CLASS II, or MINOR - Notice to Comply)
RemarksItem #
item. (IMG 5763-7)
- An open empty container of Sure Scents Hawaiian Air Freshener Crystal Beads. Marlene did not know what
happened to the beads that were once in the container, but stated if the container was open for a long period of time,
the beads would eventually evaporate. The empty container did not have a sticker on the barcode, nor any other
indication that it was a “store-use” item. (IMG 5768-9).
- An open empty container of LA’s Totally Awesome Carpet Spot & Stain Remover. Used absorbent was not found to
account for the missing liquid that was once in the container, and Marlene did not know what happened to the missing
liquid. The item did not have a sticker on the barcode, nor any other indication that it was a “store-use” item. (IMG
5770-3)
- Two empty 10 fl. oz. containers of Gain Original laundry detergent. Used absorbent was not found to account for the
missing liquid that was once in the containers, and Marlene did not know what happened to the missing liquid. The
containers did not have stickers on the barcode, nor any other indication that the items were “store-use” items. (IMG
5774-5).
-A tube of Coloreazy Color Permanent Cream Hair Color and a container of Coloreazy Developer, both for coloring
hair, were found in one bag inside the STOU HW tote. When Marlene scanned the barcode on the box in which both
items came in, the handheld scanner indicated the entire package should be placed in a STOU HW tote. However,
ammonium hydroxide was listed as an ingredient of the cream hair color and hydrogen peroxide was listed as an
ingredient of the developer. According to the Dollar Tree Hazardous Material Management poster, items containing
ammonium hydroxide are classified as Corrosive Bases (CB) and items containing hydrogen peroxide are classified as
OX. Marlene separated the cream hair color from the developer, placed the cream hair color in the CB HW tote, and
placed the developer in the OX HW tote. (IMG 5851-57, 5862-3)
- A tube of Super Glue Fix-AII Adhesive marked highly flammable on the tube’s label. Marlene placed the adhesive
into the Aerosols, Flammables, and Combustibles (AF) HW tote. (IMG 5859-61).
- An empty container of In-Wash Scent Booster beads. Marlene did not know what happened to the beads that were
once in the container. The empty container did not have a sticker on the barcode, nor any other indication that it was a
“store-use” item. (IMG 5779-80)
- A 15 fl. oz. container of White Rain Body Wash missing approximately 2-3 ounces of liquid. Although absorbent
was placed in the bag with the container, the absorbent appeared completely dry and did not appear to be used to
clean any leakage or spillage that may have released from the container. Marlene did not know what happened to the
missing liquid that was once in the container. (IMG 5783-5)
- A CB HW tote marked with accumulation start date 1/22/19 and liquid as the physical state contained the following
bagged items (IMG 5795):
- Several empty packets of GS Neutral Cleaner Concentrate (IMG 5796-7). According to Marlene, these packs are
not sold in the facility, but rather ordered by the facility to clean the facility floor.
- A 10 fl. oz. leaking container of Palmolive Oxy Power Degreaser dish soap with approximately only one ounce of
liquid left in the container. Used absorbent was not found to account for the missing liquid that was once in the
container, and Marlene did not know what happened to the missing liquid. The container did not have a sticker on the
barcode, nor any other indication that it was a “store-use” item. (IMG 5798-800)
- An AF HW tote marked with accumulation start date 1/16/19 and aerosol as the physical state contained the
following bagged items (IMG 5803):
- Two 4-in-1 Snuggle White Lavender aerosol cans, at least one of which was leaking into the bag. The bag did not
contain absorbent. According to Dollar Tree California Training Guide on Hazardous Waste Handling and Processing
Procedures, absorbent is to be applied to bagged items that are leaking and to absorb any free liquids (IMG 5804,
Page 6 of 13
FA0015376 PR0539595 SC001 05/03/2019
EHD 22-01 Rev. 09/27/2018 Small Quantity Hazardous Waste Generator OIR
1868 E.
