If the generator did not receive a copy of the manifest with the <br />handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 60 <br />days of the date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the <br />missing manifest, with some indication that the generator has not received confirmation of delivery. This information <br />shall be submitted to: <br />DTSC Report Repository <br />Generator Information Services Section <br />P.O. Box 806 <br />Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifest or prepare and submit the required information to DTSC. Submit <br />to the EHD a copy of the signed off copy of the manifest or a copy of the information submitted to DTSC. <br />This is a Class II violation. <br />301 <br />CCR 66262.34(d)(2) Facility not maintained to minimize the release of a hazardous waste. <br />One black and one red metal 55 gallon drums of used oil were observed in the hazardous waste storage area with <br />oil/oily sludge on the top of the drums. Facilities shall be maintained and operated to minimize the possibility of a <br />fire, explosion, or release of hazardous waste to air, soil, or surface water which could threaten human health or the <br />environment. Immediately clean the top of the drums and manage according to Title 22 hazardous waste <br />regulations. Submit a statement and supporting documentation explaining how this waste was managed. <br />This is a repeat violation, Class II. <br />303 <br />CCR 66262.34(d)(2) Failed to maintain all communication or alarm systems, spill control, or decontamination <br />equipment. <br />Two yellow spill containments for the used oil drums were observed with oily sludge in the bottom. All facility <br />communications or alarm systems, fire protection equipment, spill control equipment, and decontamination <br />equipment, where required, shall be tested and maintained as necessary to assure its proper operation in time of <br />emergency.
