Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />March 25, 2019 <br /> Facility Address: <br /> 2006 E Mariposa Rd, Stockton <br /> Facility Name: <br /> Family Dollar Store #11092 <br />San Joaquin County <br />Environmental Health Department <br />1868 East Hazelton Avenue, Stockton, California 95205-6232 <br />Telephone: (209) 468-3420 Fax: (209) 468-3433 Web: www.sjgov.org/ehd <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br />- A State Regulated (ST) HW tote, containing the following HW, had an accumulation start date of 5/1/18 and was <br />inaccurately marked as containing "Medicine" (IMG 5041, 5058-66, 5069): <br /> - Softee Daily Hair Dress Mango Butter <br /> - Family Wellness Hair, Skin, and Nails Biotin Dietary Supplement <br /> - Family Wellness Moisturizer Lip Balm with SPF 15 Sunscreen <br /> - Air Wick Essential Oils Refill <br /> - Softsoap Moisturizing Body Wash <br /> - A single-bagged leaking container of Suavitel fabric softener without absorbent. <br /> - An unmarked bag containing paper and an unknown red liquid. According to the StrongPak Guide to Potentially <br />Hazardous Material Management Family Dollar Version, bagged spill clean-up waste should have the name of the <br />item that spilled written on the bag before being placed in the HW tote. <br />- An open cardboard box labeled "Cough Drops" in the HW storage area contained the following items (IMG <br />5071-7): <br /> - DJZ Earphones bagged and marked "UW" for Universal Waste (UW). Randy stated this item should have been <br />placed in the UW HW tote. Randy placed the earphones in the UW HW tote. <br /> - Two aerosol cans of Hot Shot Bedbug & Flea Fogger bagged and marked "AF." Randy stated the aerosol cans <br />should have been placed in the AF HW tote. Randy placed the aerosol cans in the AF tote. <br /> - Two aerosol cans of Endust for Electronics All-Purpose Duster bagged and marked "AF." Randy placed the <br />aerosol cans in the AF tote. <br /> - A bagged Sound Logic Bluetooth Rechargeable Suction Cup Speaker, which Randy scanned and placed into the <br />UW HW tote. <br />- An aerosol can of WD-40 was found missing the actuator and located in an open container with two spray bottles <br />of cleaner. Randy Garcia, store manager, stated that the aerosol can should have been damaged out. Randy <br />scanned the aerosol can and the hand-held scanner prompted Randy to place the aerosol can into a Flammables, <br />Aerosols, & Combustibles (AF) HW tote. Randy placed the aerosol can into an AF HW tote.(IMG 4975, 4983-6) <br />- An empty container of Homeline All Purpose Cleaner was found bagged and on the floor in front of the HW <br />storage area. Randy stated he was not sure if the item had been damaged out already. Randy scanned the cleaner <br />container and the scanner prompted him to placed the cleaner into a Corrosive Bases (CB) HW tote. Randy placed <br />the cleaner container into a CB HW tote, however he did not mark the physical state, accumulation start date, nor <br />the contents on the CB HW tote (IMG 4977-8, 4988-9, 4991, 5056-7). <br />- A Glade Automatic Spray Holder was found bagged and on top of a crate adjacent the HW storage area. Randy <br />stated he was not sure if the item had been damaged out already. Randy scanned the item and the scanner <br />prompted him to place it into an AF HW tote. Randy placed the item into an AF HW tote. (IMG 4979-81, 4990-1). <br />- According to Randy, cardboard that is contaminated with leakage from damaged product is disposed of in the <br />trash. <br />- According to Araceli Padilla, an assistant store manager, absorbent/Spill Magic and paper towels used to clean up <br />chemical spills (Araceli specifically referenced laundry detergent spills) are disposed of in the trash. <br />- The Family Dollar Hazardous Waste Training Record noted the following employees as having received training <br />(IMG 5028): <br /> - Manuel Martinez <br />Page 7 of 18 <br />FA0022690 PR0539984 SC001 03/25/2019 <br />EHD 22-01 Rev. 09/22/16 Small Quantity Hazardous Waste Generator OIR
