4. Our hazardous waste vendor, Stericycle, submits manifests to the DTSC on behalf of the <br />company and has resubmitted the manifests listed here, as well as providing submission <br />information for this store (EXHIBIT C). <br />The manifests were resubmitted to DTSC on April 29, 2019, with USPS tracking number <br />7018-3090-0002-1854-6903. <br />5. Missing manifest #010788833, and missing final manifests #010868379 and 012039697 <br />have been provided to the store (EXHIBIT D). <br />6. Stericycle serviced the store on March 21, 2019, but indicated there was no waste. We are <br />in the process of rescheduling a pickup as soon as possible. As the waste for this store is <br />transported from Stockton, CA to Fernley, NV, it does fall under the 270-day storage <br />allowance. With the last pickup October 2, 2018, any waste generated immediately after <br />that pickup has likely been maintained on site approximately 180 days. <br />7. In the backroom, items have been removed to provide unobstructed access. <br />(EXHIBIT E). <br />8. All hazardous waste, both retail and store-use, is to be stored in closed containers. NOTE: <br />This includes the paint waste found in the backroom which has been processed for <br />pickup. <br />9. Containers are labeled completely and correctly. All of the old labels were removed and <br />proper labels were created with some labels showing an accumulation date of October 4, <br />2018 which is just after their last pickup (EXHIBIT F). <br />10. Weekly inspections of the hazardous waste area have resumed (EXHIBIT G). <br />11. Family Dollar store numbers had been converted when acquired by Dollar Tree. The <br />original store number was 11092. The current number is 31092. We have begun the <br />process of updating those numbers in CERs. <br />The signed certification is also included.
