Cherokee Lane
Lodi, CA 95240

Dear Ms. Florido,

In response to the inspection conducted on May 10, 2019 at Dollar Tree Store #6120 in Lodi,
California, please see the outline below, and the exhibits which follow, referencing existing
procedures, documentation and/or actions taken:

• Store management was trained that hazardous wastes are to be taken to the accumulation
area for immediate processing – not left in a “damages” or “reduced merchandise”
baskets at the front of the store. Store use items around the mop sink were properly
stickered. (See EXHIBIT A1, A2.)

The GS Neutral Cleaner Concentrate used by the store to clean the floors is not
potentially hazardous. However, we have directed the stores to process it as such to
maintain consistency and avoid confusion with the processing of similar cleaners that are
potentially hazardous.
• The Emergency Coordinator role and supporting documentation was reviewed with the
Store Manager. Her name and contact information were added to the Emergency
Response/Contingency plan as well as the San Joaquin County Emergency Contacts/FE
Locations template. The SJ County form was posted at the front of the store to be
accessible by all associates and the Emergency Response/Contingency Plan posting is in
the manager’s office (EXHIBIT B1, B2).
• A Dollar Tree EH&S Specialist retrained the Store Management team on the Company’s
Hazardous Waste Program.

They were coached on the proper identification and management of hazardous waste. All
totes containing waste were assessed for proper categorization, processing and labeling. It
was emphasized that empty and partially empty chemical containers are properly and
RECEIVED BY SJCEHD (EF) ON 7/9/2019
