Environmental Health Department <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />May 10, 2019 <br /> Facility Address: <br /> 450 S Cherokee Ln, Lodi <br /> Facility Name: <br /> Dollar Tree #06120 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br />Based on the types of violations observed, the facility employees are not trained on proper hazardous waste (HW) <br />handling or the training provided has been inadequate. <br />Several empty and partially empty chemical containers were found in the HW storage totes. When asked what <br />happened to the missing liquid from these empty containers, Andres said that the containers could have become <br />empty from either being “store-use” items to clean the facility, spilling in the store, or spilling in the shipment truck <br />during delivery to the facility. When Andres was asked about the empty containers listed below individually, he stated <br />he did not know what specifically happened to the missing liquid from each container, unless the container was <br />marked with an orange sticker on the item’s barcode, indicating the item was a “store-use” item. At the time of <br />inspection, a partially empty container of Spic and Span Antibacterial Cleaner was observed on the facility’s cardboard <br />bailer and marked with an orange “store-use” sticker on the item’s barcode (IMG 6093-4). <br />The following observations were made at the time of inspection: <br />- An Aerosols, Flammables, & Combustibles (AF) HW tote was found lacking the physical state on the HW label (IMG <br />6066). Andres marked the physical state on the AF HW tote at the time of inspection. <br />- An open 6 fl. oz. container of hydrogen peroxide was found in a Dollar Tree basket labeled “Damages” at the front of <br />the facility. When Andres scanned the item’s barcode, the hand held scanner indicated the item should be placed in <br />an State-Regulated, Toxics, Electronics, OTC Pharmaceuticals, Universal Waste (STOU) HW tote. However, <br />according to the Dollar Tree Potentially Hazardous Materials Management poster, posted on a wall next to the HW <br />storage area, items containing hydrogen peroxide are classified as Oxidizers (OX). Andres bagged the item with <br />absorbent and placed the hydrogen peroxide into an OX HW tote. (IMG 6062, 6065, 6070-2, 6128-9). <br />- A Corrosive Bases (CB) HW tote was found containing three empty 20 fl. oz. containers of LA’s Totally Awesome All <br />Purpose Concentrated Cleaner & Degreaser Spot Remover, free liquid from the containers, and saturated absorbent, <br />bagged and labeled as CB HW. The amount of liquid and absorbent in the bag did not account for the amount of liquid <br />that was missing from the containers. None of the containers were marked with an orange “store-use” sticker. (IMG <br />6073-5). <br />- The CB tote containing the three empty containers of LA’s Totally Awesome All Purpose Concentrated Cleaner & <br />Degreaser Spot Remover, was found broken on one side of the tote. Andres transferred the HW bag containing the <br />cleaner into, and the CB HW label onto, an unbroken tote. (IMG 6076) <br />- The following items were found in a STOU HW tote with accumulation start date 4/26/19 (IMG 6077-84, 6091-2): <br /> o A broken nail polish container labeled AF HW. The hand-held scanner stated the nail polish should be placed in an <br />AF HW tote. Andres placed the nail polish into an AF HW tote. <br /> o An open, partially filled, container of green slime labeled HW. <br /> o An empty container of Sure Scents Pumpkin Air Freshener Crystal Beads, labeled STOU HW, and not marked <br />with an orange “store-use” sticker. <br /> o Three partially empty containers of dried paint labeled HW. <br />- The following items were found in a STOU HW tote with accumulation start date 5/8/19 (IMG 6085-90): <br /> o An open leaking container of Cinnamon Spice Fragrance Diffuser, labeled STOU HW, and not marked with an <br />Page 7 of 13 <br />FA0023050 PR0540455 SC001 05/10/2019 <br />EHD 22-01 Rev. 09/27/2018 Small Quantity Hazardous Waste Generator OIR <br />1868 E.
