Uniform Hazardous Waste Manifest <br />(UHWM) numbers 012218722 FLE ship date 05-07-19 and 012218721 FLE ship date 05-07-19 were found without <br />a signed copy from the designated facility. <br />REGULATION GUIDANCE: <br />(a) A generator who does not receive a copy of the manifest with the handwritten signature of the owner or operator <br />of the designated facility within 35 days of the date the waste was accepted by the initial transporter shall contact the <br />transporter and/or the owner or operator of the designated facility to determine the status of the hazardous waste. <br />(c) A hazardous waste generator who does not receive a copy of the manifest with the handwritten signature of the <br />owner or operator of the facility to which the generator's waste is submitted, within 60 days from the date that the <br />hazardous waste was accepted by the initial transporter, shall submit to the department a legible copy of the <br />manifest, with some indication that the generator has not received confirmation of delivery. <br />(d) Generators shall submit the information to the department at: DTSC Report Repository, Generator Information <br />Services Section, P.O. Box 806, Sacramento, CA 95812-0806 <br />CORRECTIVE ACTION: Locate a copy of the destination facility signed manifests and send a copy to the San <br />Joaquin County Environmental Health Department (EHD). If the destination facility signed manifests cannot be <br />found then send to DTSC a copy of each generator signed manifest and a statement that the designated facility <br />signed copy was not received by the generator. DTSC address is DTSC Report Repository, Generator Information <br />Services Section, PO Box 806, Sacramento, CA 95812-0806. Provide a corrective action statement to the EHD <br />within 30 days. <br />This is a Class II violation. <br /> 304 CCR 66262.34(d)(2) Failed to maintain adequate aisle space for unobstructed response to emergencies. <br />OBSERVATION: The following area does not have adequate aisle space: <br />- The hazardous materials storage bin situated adjacent to the North entrance of the facility has inadequate aisle <br />space. <br />REGULATION GUIDANCE: The owner or operator shall maintain aisle space to allow the unobstructed movement <br />of personnel, fire protection equipment, spill control equipment, and decontamination equipment to any area of <br />facility operation in an emergency, unless it can be demonstrated to the Department that aisle space is not needed <br />for any of these purposes. <br />CORRECTIVE ACTION: Immediately maintain aisle space to allow the unobstructed movement of personnel, fire <br />protection, spill control equipment, and decontamination equipment to any area of facility operation in an <br />emergency.
