Environmental Health Department <br />AMENDED <br />10/18/2019 <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />October 11, 2019 <br /> Facility Address: <br /> 2505 W HAMMER LN, STOCKTON <br /> Facility Name: <br /> SUTTER GOULD <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 403 CCR 66262.34(d)(2) Failed to keep hazardous waste containers closed except when adding or removing hazardous <br />waste. <br />OBSERVATION: The following open hazardous waste storage containers were observed on-site: <br />- one approximately 15 gallon RCRA Hazardous Waste container located in radiology room 135 was open <br />- one approximately 3 gallon RCRA Hazardous Waste container located in pediatrics room 1079 was open <br />- one approximately 3 gallon RCRA Hazardous Waste container located in internal medicine room 2070 was open <br />- one approximately 3 gallon RCRA Hazardous Waste container located in family medicine room 3065 was open <br /> <br />REGULATION GUIDANCE: A container holding hazardous waste shall always be closed during transfer and <br />storage, except when it is necessary to add or remove waste. <br />CORRECTIVE ACTION: Immediately close these containers and ensure all hazardous waste containers are closed <br />when not adding or removing waste. Provide a corrective action statement and supporting documentation to the <br />San Joaquin County Environmental Health Department within 30 days. <br />This is a minor violation. <br /> 605 CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br />OBSERVATION: Hazardous waste containers were observed on-site with incomplete labeling. The containers <br />were: <br />- one approximately 15 gallon RCRA Hazardous Waste container in the hazardous materials storage bin was <br />lacking generator name and address and composition <br />- one approximately 15 gallon RCRA Hazardous Waste container hazardous materials storage bin was lacking <br />generator name and address and composition <br />- one approximately 15 gallon RCRA Hazardous Waste container hazardous materials storage bin was lacking <br />accumulation start date, generator name and address, hazardous properties, composition, and physical state <br />- one approximately 15 gallon RCRA Hazardous Waste container containing barium sulfate waste in radiology room <br />135 was lacking generator name and address, hazardous properties, composition, and physical state <br />- one approximately 3 gallon RCRA Hazardous Waste container containing sani wipes and waste hand sanitizer <br />bottles in pediatrics room 1079 was lacking generator name and address, hazardous properties, composition, and <br />physical state <br />- one approximately 3 gallon RCRA Hazardous Waste container in internal medicine room 2070 was lacking <br />generator name and address, hazardous properties, composition, and physical state <br />- one approximately 3 gallon RCRA Hazardous Waste container containing waste testosterone in family medicine <br />room 3065 was lacking generator name and address, hazardous properties, composition, and physical state <br />REGULATION GUIDANCE: All hazardous waste containers shall be marked with the following information: the <br />words “Hazardous Waste”, name and address of generator, hazardous properties, physical state, composition <br />(contents), accumulation start date <br />CORRECTIVE ACTION: Immediately label the containers as required for hazardous waste.
