The facility implements a throughout incident <br /> investigation process and investigates near-misses and minor incidents. The team <br /> reviewed the incident reports on file and concluded that the incidents were <br /> addressed appropriately and confirmed actions were taken to prevent <br /> reoccurrence. None of the incident scenarios were included in the PHA what-if <br /> questions. A few incidents are described below. <br /> On 7/12/16 there was a release of approximately 47 pounds of ammonia to the <br /> diffusion tank.
