Process Hazard Analysis Report July 2020
The motivation for the change was to automate a manual process. The contractor
installed dual pressure regulator, globe shut off valve, and an angle shut off valve
on CH1 And CH2. Before this modification, personnel would close SVA-S 100, a
4" Danfoss manual valve. Be making this modification, the electronic valve SVA-S
80, a 3" Danfoss globe valve, will automatically close to prevent the chillers CH1
and CH2 from freezing up.
MOC 0002 — Condenser CD line routing improvement (System 2)
The motivation for the change was initiated by a mechanical integrity audit finding
and PHA recommendation. The modification changed how the condenser drain
(CD) lines exit the condenser and collect into a manifold pipe that terminates at the
HPR inlet. Isolation valves were added to be able to pump down the condenser
properly and isolate from the system. Purge valves were also added to be able to
properly purge the system for non-condensibles.
MOC 0003 — Relief piping routing improvement (System 2)
The motivation for the change was initiated by a mechanical integrity audit finding
and PHA recommendation. This modification was made to ensure compliance with
ANSIMAR 2. The discharge piping for four (4) pressure relief valves were
extended to be 7'-4" above the highest working platform; e.g. tanks 178-183. The
extended piping is supported by the tank wall.
MOC 0004 —Addition of charging station to HPR (System 2)
The motivation for the change was initiated by a mechanical integrity audit finding
and PHA recommendation. A charging station was added to the HPR.
MOC 0005 —Addition of oil drain lines on chillers 3 & 4 (System 2)
The motivation for the change was initiated by a mechanical integrity audit finding
and PHA recommendation. The modification was a process improvement by
providing an improved means for draining oil from the chillers. The previous
configuration had the potential to trap oil in the piping.
MOC 0006 — HSD line relief valve (System 2)
The motivation for the change was initiated by a mechanical integrity audit finding
and PHA recommendation. A relief valve from the high side discharge (HSD) line
was removed per mechanical integrity audit.
13
