TABLE 4 <br /> Spill Response Notification Procedures —§112.7(a)(4) <br /> UNLESS NOTED OTHERWISE, NOTIFICATIONS ARE TO BE MADE ONLY BY THE <br /> ENVIRONMENTAL SPECIALIST AND ONLY AFTER THE EMERGENCY CONDITIONS <br /> RELATED TO THE RELEASE HAVE BEEN IMPLEMENTED. THIS RESTRICTION IS <br /> NECESSARY TO PREVENT MISCOMMUNICATION AND TO ASSURE THE NOTIFICATIONS <br /> ARE PROPERLY CONDUCTED. <br /> All spills shall be reported to the Environmental Specialist. <br /> NOTE: If the Environmental Specialist is unable to make the notifications, the Environmental <br /> Health and Safety Manager will complete the notifications. <br /> Spills That Have Reached. or Have A Potential to Reach Surface Waters <br /> 1. Call the National Response Center at 1-800-424-8802. State as follows: <br /> a. This is a spill report. <br /> b. Give your name and the company name. <br /> C. Give location of spill (Name of city and state.) <br /> d. Describe nature of spill, type of product, and estimate size of spill. If accurate, <br /> note that "It is believed" that the spill has not and may not reach surface waters. <br /> e. Describe type of action taken thus far, type of assistance or equipment needed. <br /> 2. Contact the California State Warning Center (Cal OES Warning Center) at (800) 852- <br /> 7550 or(916) 845-8911. <br /> 3. Notify the Owens-Brockway Environmental Specialist (Table 5). <br /> Be prepared to provide the type of material, quantity, and the potential for soil or <br /> groundwater impact. State any contact you have had with non-company personnel, <br /> reporting you have done, or believe may be appropriate, response and corrective actions <br /> taken, and resources that may be required. <br /> 4. For releases of over 1,000 gallons, or more than two releases greater than 42 gallons <br /> ea. within a 12-month period that cause a sheen on surface waters or shoreline, or <br /> sludge or emulsion in waters, submit the SPCC Plan to the Regional US EPA office. <br /> Provide a description of the spill and detail corrective action taken to prevent a <br /> recurrence. <br /> 5. Document the following information: <br /> a. The agency contacted, individual accepting the report, and the time the report was <br /> provided. <br /> b. Incident number if assigned. <br /> c.
