Oily staining was observed on the ground next to the used oil tank located in HWA-10 on the site map (IMG <br /> 6938-40). <br /> 3. Oily staining was observed on the ground,walls, and pillars throughout the lower ground/basement area of the <br /> facility(IMG 6947-9). <br /> 4. Oily staining and used absorbent pads were observed on the ground throughout the area next to the satellite <br /> accumulation area and vac pumps#2 and#3, located in the lower ground/basement area of the facility(IMG 6950-60). <br /> 5.A drain pan containing what appeared to be oily liquid was observed beneath a pipe on vac pump#3(IMG 6954-55). <br /> 6. Visible discharge, staining, and used absorbent pads were observed on the ground around the base of the diesel <br /> belly tank(IMG 6964-6). <br /> 7. Visible discharge was observed coming out of a pipe,which was coming from the room containing the C3 basement <br /> vacuum. The discharge was falling onto the ground soil beneath the pipe.A large area of the ground soil was <br /> contaminated with the discharge.According to Bill,the discharge was vacuum pump oil.A drum of the oil was <br /> observed inside the room containing the C3 basement vacuum (IMG 6970-82).According to Bill,the leaking machinery <br /> in this area would not be used until fixed, and he would have on site personnel begin the clean-up of the contaminated <br /> soil. <br /> 8. Oily staining and used absorbent was observed on the concrete pads beneath the machinery in C3 basement <br /> vacuum room (IMG 6982-88). <br /> The following observation was made during the 2017 hazardous waste inspection, and has yet to be formally resolved: <br /> 1. Oily spillage was observed in the Batch House area under a mixing equipment and inside the secondary <br /> containment pan located under the said equipment. The EHD received a corrective action statement and supporting <br /> documentation explaining how this waste was managed on 6/13/2019 from Bill Boscacci.This issue has been <br /> corrected. <br /> Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous <br /> waste to air, soil, or surface water which could threaten human health or the environment. <br /> Immediately fix any leaks and clean up any staining and used absorbent, including the items stated above, and <br /> manage according to Title 22 hazardous waste regulations. <br /> Submit a statement and supporting documentation explaining how this waste was managed. <br /> This is a repeat violation, Class ll. <br /> FA0006674 PR0220086 SCO01 06/05/2019 <br /> EHD 22-03 Rev.09/27/2018 Page 8 of 13 RCRA Large Quantity Hazardous Waste Generator OIR <br /> 1868 E.
