For instance,if you are <br /> the Permit Holder submit all three insurance forms;however, if you are transporter only, <br /> submit the Certification of Commercial Auto/Trucking Liability Insurance(Addendum Q. <br /> DCharges are based on full load hauling capacity of the tanker. The hauling company must <br /> submit for each tanker the license number,hauling capacity and internal identification <br /> number to receive an EBMUD decal. An EBMUD decal is needed prior to entering the plant <br /> for disposal. To coordinate a decal,contact the EBMUD Field Coordinator at 510/287-1630 <br /> or 510/385-6158. Please review form attached and update. <br /> DRIVER REQUIREMENTS <br /> All new drivers must schedule and conduct a site orientation to confirm understanding of <br /> potential plant hazards,rules of conduct and discharge procedures to satisfy EBMUD's CA <br /> OSHA Cal STAR audit, Security, and operations requirements. Please schedule an <br /> orientation for drivers that have an"N"indicated on the form,with the EBMUD Field <br /> Coordinator at 510/287-1630 or 510/385-6158 <br /> TANKER REQUIREMENTS <br /> Tankers discharge line must have a 4-inch male camlock adaptor. <br /> F]Tankers must have a back-up horn alarm safe, device. <br /> Tankers must have an affixed EBMUD decal. There is no charge for decals. <br /> The EBMUD Wastewater Treatment Plant is located at 2020 Wake Avenue, Oakland. See map <br /> enclosed for directions.
