i <br /> TRUCKED NON-HAZARDOUS WASTE DISPOSAL PERMIT <br /> �3 Terms and Conditions <br /> EBMUD Addendum E <br /> Trucked Waste Delivery Procedures <br /> GENERAL INFORMATION <br /> The EBMUD Wastewater Treatment Plant is located at 2020 Wake Avenue, Oakland. The Plant is open 24 <br /> i hours/day, 7 days/week. <br /> 1HAULING COMPANY,DRIVER AND TANKER REQUIREMENTS <br /> 1. Must have all hauling company contact information, drivers names, and appropriate insurance requirements. <br /> 2. Charges are based on full load hauling capacity of the tanker. The hauling company must submit for each <br /> tanker the license number,hauling capacity and internal identification number. <br /> 3. Drivers must receive a Site Orientation prior to each load delivery. <br /> 4. Tankers discharge line must have a 4-inch male camlock adaptor and a back-up horn alarm safety device. <br /> 5. Tankers must have an affixed EBMUD issued decal. <br /> 6. The main plant entrance has a live Rail Road line. Do not wait or park on the tracks at any time. <br /> PLANT HAZARDS <br /> 1. NO SMOKING (on-site Safety Hazard as there are flammable gases, oxygen and methane present). <br /> 2. Construction activities. When overhead activities are occurring hard hats(available) are required. <br /> 3. Infectious materials are present at wastewater treatment plant and managed in wastewater treatment process. <br /> 4. In the event you hear an emer�enc alarm of six long tones, call ext. 1522 or contact District staff for further <br /> instructions. <br /> 5. During transportation of the waste material, degradation of the waste material may occur inside the tanker <br /> and build up gases that may have Immediately Dangerous Life and Health concentrations present when the <br /> driver vents the tanker. <br /> RULES OF CONDUCT <br /> 1. Speed Limit is 10 miles per hour. <br /> 2. If you find a spill upon arrival report it to the shift supervisor at 287-1522(when using on-site phone, dial <br /> 1522)below before off-loading. <br /> 3. Driver must be in compliance with all OSHA regulations, in particular fall protection per Title 8 CA Code <br /> Regulations Sections 3210(b)and 5155. Driver must not break plane of trailer(i.e., do not put your head <br /> inside hatch). <br /> 4. Do not stand on tanker without OSHA specific fall protection. <br /> 5. Drivers must stay with truck during discharge. <br /> 6. All waste must be discharged through hose directly to pipe. Do not disconnect site discharge hose from <br /> discharge pipe. Do not discharge from the trailer directly to the wash down trench. <br /> 7. Drivers are responsible for wash down of spills. <br /> 8. No exterior truck washing is permitted. <br /> 9. Do not leave any trash or debris at the discharge area. <br /> 10.Do not drink or wash hands with wash down water as its non-potable water! <br /> DISCHARGE INSTRUCTIONS <br /> 1. Each delivery must be accompanied with the appropriate approved Waste Acceptance Agreement. A copy <br /> is to be left with the guard upon checking in at the EBMUD Gate. <br /> 2. Review your issued ticket to verify all items are correct and proceed to the for the appropriate discharge <br /> location (see map on ticket). <br /> 3. Back tanker into available receiving station truck bay. <br /> 4. Attach the tanker's male camlock fitting to the provided hose with the female camlock. <br /> 5. When discharge(gravity or truck pressured) is complete, empty all the liquid from the hose by rolling it on <br /> the ground, and then replace it on the free stand-rack. <br /> 6. Use hose provided to rinse area, and then wrap hose on rack. <br /> 7.
