The following is an itemized list of hazardous waste violations that have not been <br /> addressed for AVIVA PERFORMANCE as of October 02, 2020. <br /> Open violations from August 13, 2020 inspection <br /> Violation#115-Failed to retain copy of manifest or bill of lading for spent lead acid batteries for 3 years. <br /> OBSERVATION: Bills of lading for the recycling of lead acid batteries from 2018 to present were not found on site. <br /> REGULATION GUIDANCE: <br /> (A)The generator shall use either the manifest or a bill of lading which fulfills the requirements of Title 13 CCR <br /> section 1161,to record the shipment of spent lead-acid storage batteries to a person who stores the batteries or <br /> who uses, reuses, recycles or reclaims the batteries or their components. <br /> (B)The generator shall retain, at the generator's place of business for at least three years, a legible copy of each <br /> manifest or bill of lading which identifies spent lead-acid storage batteries shipped to a person who stores the <br /> batteries or who uses, reuses, recycles or reclaims the batteries or their components. <br /> CORRECTIVE ACTION: Provide copies of the last three battery core bill of lading records and a corrective action <br /> statement to the San Joaquin County Environmental Health Department within 30 days. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#303-Failed to maintain all communication or alarm systems,spill control,or decontamination <br /> equipment. <br /> OBSERVATION:The eyewash bottles in the hazardous waste area are expired with a date of 8/19. <br /> REGULATION GUIDANCE:All facility communications or alarm systems,fire protection equipment, spill control <br /> equipment, and decontamination equipment,where required,shall be tested and maintained as necessary to assure <br /> its proper operation in time of emergency. <br /> CORRECTIVE ACTION: Bring the eyewash station into proper working order by replacement with new eyewash <br /> bottles.
