Our hazardous waste vendor, Stericycle, submits manifests to the DTSC on behalf of the <br /> company and has resubmitted the manifest as well as providing a submission letter for <br /> this store. Please see the vendor letter of responsibility(EXHIBIT C). <br /> As this store opened in April 2018, the store had one pick-up on October 2, 2018. <br /> Stericycle went back for a second pick-up on March 24, 2019, but indicated there was no <br /> waste. We are scheduling the next pickup ASAP. As the waste for this store is <br /> transported from Stockton, CA to Yuma, AZ, it does fall under the 270-day storage <br /> allowance. With the first pickup October 2, 2018, any waste generated soon after that <br /> date has likely been maintained on site approximately 180 days. <br /> 5. Missing manifest#012039698 FLE has been provided to the store (EXHIBIT D). <br /> The generator manifest copy was resubmitted to DTSC by Stericycle on April 26, 2019 <br /> with USPS tracking number 7018-3090-0002-1854-6903. <br /> 6. Containers are labeled completely and correctly. Old labels were removed and proper <br /> labels were created showing an accumulation date of October 3, 2018 which is one day <br /> after the last pickup (EXHIBIT E). <br /> 7. In the backroom, items blocking aisles were removed to provide unobstructed access to <br /> exits and electrical panels as well as the hazardous waste accumulation station <br /> (EXHIBIT F). <br /> 8. The Store Manager has resumed weekly inspections of the hazardous waste area <br /> (EXHIBIT G). <br /> 9. Family Dollar store numbers had been converted when acquired by Dollar Tree. The <br /> original store number was 12078. The current number is 32078 as shown in DTSC's <br /> system. However, the Family Dollar numbers in CERs are the old numbers. We have <br /> begun the process of updating those numbers. <br /> 10. Our guidelines for marking down potentially hazardous items: <br /> Useable items with minor damage may be marked down for sale at a reduced price. <br /> "Minor damage" can be items with crushed corners or tears that can be taped, some items <br /> with partial content, or a single missing piece (for example, packages of batteries or tea <br /> lights where one item has been removed). <br /> However, an item that is potentially hazardous is considered waste if it cannot be used for <br /> its intended purpose, is damaged beyond simple repair, cannot be sold or recycled, and <br /> must be disposed of(for example, an aerosol can missing the actuator or a container of <br /> dried-out multipurpose wipes). <br /> See the excerpt from the "Quick Reference Guide," a separate attachment, (EXHIBIT H). <br /> May 1,2019 pg.2 <br />
