San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.sogov.org/ehd <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> FAMILY DOLLAR#32078 66 S WILSON WAY, STOCKTON March 20, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> HW tote the item should be placed in (IMG 4950-4951). <br /> -An empty 20 fl.oz. container of Palmolive Vinegar&Lemon Dish Liquid,for which the scanner did not indicate <br /> which HW tote the item should be placed in (IMG 4956-4957). <br /> -An empty 21.6 fl. oz. container of Gain Ultra Original dish soap,for which the scanner indicated the item to be <br /> placed in the TX tote(IMG 4952-4953). <br /> -An empty 33 fl.oz. container of Homeline Window&Glass Cleaner,for which the scanner indicated the item to <br /> be placed in the ST tote(IMG 4954-4955). <br /> -An open 56 fl. oz. container, 1/3 full, of Dawn Ultra Antibacterial Hand Soap,for which the scanner indicated the <br /> item to be placed in the AF tote(IMG 4959-4961). <br /> -A Partially filled containers of Nyco All Purpose Degreaser(IMG 4906,4911). <br /> -A filled container of window cleaner(IMG 4906,4911). <br /> -The bottom of the bucket contained leakage of an unknown white/cream colored liquid (IMG 4906,4911). <br /> When asked how employees determine which hazardous waste tote an item should be placed in when the <br /> hand-held scanner does not provide any indication, nor is the item or a similar type of item listed on the Family <br /> Dollar hazardous materials categories poster(IMG 4922), Micheal stated he places the item on a shelf in the HW <br /> storage area for the waste hauler to sort when the HW is picked up. Michael was not familiar with the phone number <br /> listed on the poster, nor the phone number listed on the Material Safety Data Sheet(MSDS)page in the HW binder <br /> (IMG 4962),to call if questions came up about an item's HW determination. Michael also did not know about the <br /> MSDS link on the store portal to find SDS product information. <br /> -An empty 1 qt. container of American XT Premium Sae 1 OW-30 Synthetic Blend Motor Oil was found in a pile of <br /> trash on the ground adjacent the facility's dumpster.At the time of inspection,the facility's dumpsters were <br /> accessible to the public.According to Michael, he has made several attempts since December 2018 to have the <br /> fence repaired, in order to prevent the public from dumping their trash into the facility's dumpsters, but to no avail. <br /> (IMG 4927-4929) <br /> The following was found on shelves behind and adjacent to the front register: <br /> -An empty open blue basket labeled "DAMAGE!"A box of empty plastic bags was found on the shelf above the <br /> basket.According to Michael, damaged items from the store are placed in this basket and are processed once a <br /> week, at which point the items are placed in a plastic bag and then placed in a hazardous waste tote. (IMG <br /> 4930-4933) <br /> -An open 12 fl. oz., 1/5 full, container of Modesa Strengthening Nail Polish Remover.According to Michael, the nail <br /> polish remover may have been used to clean up spilled nail polish on the facility floor, however no nail polish spill <br /> clean-up waste was observed at the time of inspection. (IMG 4934-4935) <br /> -A bottle of hydrogen peroxide was found bagged and labeled "OX."According to Michael, he had not had a chance <br /> to take the hydrogen peroxide to the hazardous waste storage area.
