Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />March 27, 2019 <br /> Facility Address: <br /> 2043 JACKSON AVE, ESCALON <br /> Facility Name: <br /> FAMILY DOLLAR #32181 <br />San Joaquin County <br />Environmental Health Department <br />1868 East Hazelton Avenue, Stockton, California 95205-6232 <br />Telephone: (209) 468-3420 Fax: (209) 468-3433 Web: www.sjgov.org/ehd <br />- When asked if the facility had any items that have been recalled, Kassandra stated the facility has been <br />instructed, via email, to pull the following items off the store shelves and not to sell them until further instruction. <br />A tote was found in the store office containing the following items (IMG 5157-65): <br /> - Three boxes of Prevacid 24 HR 14-Capsules <br /> - Six boxes of Orajel Instant Relief for Teething Pain <br /> - One box of Preparation H Ointment <br /> - Five boxes of Infants’ Ibuprofen <br /> - Maximum Strength Anti-Fungal Nail Pen <br />Kassandra stated the items have been in the tote since last year. Kassandra could not provide any <br />documentation explaining why the items cannot be sold. <br />Submit a statement to the EHD explaining Family Dollar's management of recalled items and explain how the <br />items mentioned above will be managed. <br />- An alarm clock with an AC/DC power adapter was found on the ground adjacent the facility’s dumpster (IMG <br />5135). According to Jaime Muima, assistant manager, homeless people often dump their trash in the facility’s <br />dumpster. Jaime removed the alarm clock from the area and placed it on the counter at a front register in the <br />facility. Kassandra instructed Jaime to bag the alarm clock and place it in the Universal Waste tote in the <br />hazardous waste storage area. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by 5/12/2019. <br />Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
