<br /> The permit application to install an underground storage tank ( UST) system at the above address was
<br /> received by the San Joaquin County Environmental Health Department ( EHD ) on October 6 , 2017 . In
<br /> order for your application review to continue , please provide the following additional information (three
<br /> copies each ) and address the following comments :
<br /> 1 ) Page 3 of the Application for Underground Storage Tank Installation Permit : Indicate the
<br /> Contractor' s name , phone , address , license number, hazardous waste certificate , worker' s
<br /> comp number, proposed install date , and the plan reviewer' s name and date .
<br /> 2 ) Provide UPCF Underground Storage Tanks - Facility form (A) with facility information and
<br /> property owner information in a legible format .
<br /> 3 ) Form B tank lists AN alarm and flapper, however, flapper only is identified on the permit
<br /> application . Clarify and resubmit .
<br /> 4 ) Provide current contractor' s training certifications for installation and testing of all proposed
<br /> UST equipment/components , including but not limited to : tanks , sumps , sensors , monitoring
<br /> panel , leak detectors , piping , spill containment , and penetrations .
<br /> 5 ) Provide a drawing /map indicating the locations of all water wells , septic systems , and storm
<br /> drains .
<br /> 6 ) Submit a work plan for enhanced leak detection ( ELD ) testing , to be conducted by a properly
<br /> licensed individual .
<br /> 7 ) Submit manufacturers ' cut sheets for tanks , piping , and equipment . (e . g . Tank cut sheet for
<br /> specific tank model used , electronic leak detector, flex connectors , under dispenser
<br /> containment , vent transition sumps , fill sumps , all product penetrations with the correct model
<br /> number, brine , secondary pipe sensor , spill containers , and tank interstitial sensor) . Cut sheets
<br /> for items missing on the install plan pages M . 5 . 3 . 01 and M . 5 . 3 . 02 are #28 , 41 , 51 - 54 , 72 , 73 ,
<br /> 811 821 85 , 90 ( indicate which specific dispenser island ) , 102 , 104 ( be specific) , 105 - 111 , 150 ,
<br /> 153 , 163 , 166 , 171 - 176 , 200 , 204 , 205 , 207 , 208 , 211 , 212 , and for items # 143 , 146 , 152 , 156 ,
<br /> 161 , 164 the correct model number should be indicate and also provide cut sheets if not
<br /> available . Please provide manufacturer' s cut sheets for parts listed on G . 0 . 5 of the install plans .
<br /> 8 ) Provide UL Listing/third party testing approval for all UST system equipment .
<br /> 9 ) Provide the accurate overfill prevention model number and cut sheet . Provide drop tube
<br /> measurement calculations .
<br /> 10 ) Sheet M . 5 . 3 . 02 lists two liquid sensor options ( 208 and 323 position sensitive) for sump
<br /> monitoring , one should be selected .
