Environmental Health Department <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />April 25, 2019 <br /> Facility Address: <br /> 2318 S AIRPORT WAY, STOCKTON <br /> Facility Name: <br /> FAMILY DOLLAR #31867 <br />- Three boxes of Prevacid 14CT Capsules were found in the store's office (IMG 5613). According to Monica, the <br />Prevacid was an item that store employees were directed to remove from the sales shelves and hold until further <br />notice by corporate. According to Family Dollar documentation dated 10/4/2018, the "Pull and hold" of SKU <br />914127 Prevacid 14CT Capsule was assigned back on 10/3/2018 (IMG 5614). Provide the EHD with an <br />explanation of how the three boxes of Prevacid will be managed. <br />- Seven boxes of Infant's Ibuprofen were found in the store's office (IMG 5613). According to Monica, the ibuprofen <br />was an item that store employees were directed to removed from the sales shelves and hold until further notice <br />by corporate. According to Family Dollar documentation dated 12/6/2018, the "Pull and hold" of SKU 902344 FW <br />Infant Ibuprofen Drop Berry .5FL OZ was assigned back on 12/5/2018 (IMG 5615). Provide the EHD with an <br />explanation of how the seven boxes of Infant's Ibuprofen will be managed. <br />- Met Sherry Aebersold, Family Dollar & Dollar Tree Environmental Health and Safety Compliance Manager, at the <br />time of inspection. Sherry stated on 5/7/2019 Family Dollar & Dollar Tree employees in the surrounding area will <br />be provided hazardous waste training at the Dollar Tree Distribution Center located in Stockton, CA. <br />An inspection checklist was provided to the facility operator on the day of inspection. The EHD has amended <br />and written the complete report which replaces the initial checklist on 5/6/2019. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by 6/5/2019. <br />Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
