A hazardous waste generator shall not treat, store, dispose <br />of, transport or offer for transportation, hazardous waste without a hazardous waste ID number. Go to the DTSC <br />web site http://www.dtsc.ca.gov/IDManifest/PERMHWID.cfm for the form to reactivate the ID number . <br />Immediately reactivate your state ID number and submit evidence to the EHD. <br />This is a repeat violation, Class II. <br /> 104 CCR 66262.34(d)(2) No modified contingency plan. <br />A modified contingency plan is lacking. There must be at least one emergency coordinator on site or on call to <br />coordinate emergency response measures, and the following information must be posted by a phone: the name and <br />phone number of the emergency coordinator; location of fire extinguishers, spill control equipment, and if present, <br />fire alarm; and the phone number of the fire department, unless the facility has a direct alarm. Immediately post the <br />required information by a phone. A form is provided that can be used for this purpose. <br />This is a repeat violation, Class II. <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br />At the time of inspection, it could not be demonstrated (not necessarily documented) that employees who handle <br />hazardous waste were properly trained. According to Bulmaro, not all of the the employees have been trained in <br />managing hazardous waste. The generator must ensure that all employees who handle hazardous waste are <br />thoroughly familiar with proper waste handling and emergency procedures. Provide proof of training to the EHD for <br />employees whose responsibilities include hazardous waste. <br />This is a repeat violation, Class II. <br /> 110 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br />Copies of uniform manifests for the disposal of hazardous waste for 2015, 2016, 2017, and 2018 were not found on <br />site. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all <br />receipts used in a consolidated manifesting procedure on site for three years and have them readily available for <br />review. Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and <br />submit copies to the EHD. <br />This is a Class II violation. <br /> 113 HSC 25160.2(b)(3) Failed to keep copies of consolidated manifesting receipts for three years. <br />Copies of hazardous waste disposal records for the past three years were not found on site. Hazardous waste <br />generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a <br />consolidated manifesting procedure on site for three years and have them readily available for review.
