(continued) facility to include all of the necessary information and submit a copy of the updated plan <br /> to the EHD by December 16, 2011. <br /> 113. No emergency coordinator has been identified for this facility. At all times there shall be at least <br /> one employee either on the facility premises or on call (i.e., available to respond to an emergency by <br /> reaching the facility within a short period of time) with the responsibility for coordinating all emergency <br /> response measures. This emergency coordinator shall be thoroughly familiar with all aspects of the <br /> facility's contingency plan, all operations and activities at the facility, the location and characteristics of <br /> waste handled, the location of all records within the facility, and the facility layout. In addition, this <br /> person shall have the authority to commit the resources needed to carry out the contingency plan. <br /> Immediately appoint an emergency coordinator for this facility. <br /> 117. A site specific training program and training records were not available at the facility. Facility <br /> personnel shall complete a training program that teaches them to perform their duties in a way that <br /> ensures the facility's compliance with Title 22 regulations. This program shall be directed by a person <br /> trained in hazardous waste management procedures and shall include the following.. <br /> - hazardous waste management procedures relevant to the positions in which they are employed <br /> - contingency plan implementation <br /> - emergency procedures, equipment, and systems <br /> - key parameters for automatic waste feed cut-off systems <br /> - communications or alarm systems <br /> - response to fire, explosion, or groundwater contamination incident <br /> - shut down of operations <br /> The training program shall be completed by each employee within six months of hire, prior to <br /> unsupervised work, and shall be reviewed annually. Training records shall be maintained on site until <br /> closure of the facility and shall include: <br /> - name &job title of each employee <br /> - written job description for each position (job title) listed, including requisite skill, education, <br /> qualifications, and duties <br /> - description of training to be given to each position <br /> - records that training was completed for each employee <br /> Immediately prepare and implement a complete hazardous waste management training program for this <br /> facility in accordance with Title 22 regulations and ensure that all employees complete this training within <br /> six months of hire and at least annually thereafter.
