x Environmental Health Department <br /> SANJUUII <br /> 6A� —COUNTY <br /> `c v Greatrxess grows here, <br /> Underground Storage Tank Program Compliance Review Report <br /> Facility Name and Address: Harney Lane 76, 141 E HARNEY LN, LODI 95240 <br /> #214 CCR 2712 Failure to comply with UST operating permit conditions <br /> OBSERVATIONS:OBSERVATION:The facility is not in compliance with the written conditions of the operating permit. <br /> -A complete CERS submittal has not been made. <br /> REGULATION GUIDANCE: The owner and operator of underground storage tank(UST)systems shall comply with all <br /> requirements of the permit issued for the operation of the UST system. <br /> CORRECTIVE ACTION: Immediately take all necessary actions to bring these UST systems into compliance. <br /> This is a Class II violation. <br /> #313 HSC 25290.1(d) Monitoring system incapable of detecting entry into secondary containment(post-Jul 2004) <br /> OBSERVATIONS:OBSERVATION:A piping smart sensor was found in no vacuum alarm on 11/10/20. <br /> REGULATION GUIDANCE: The continuous monitoring system shall be maintained to detect the entry of the liquid or vapor <br /> phase of the hazardous substance stored in the primary containment into the secondary containment, and to detect water <br /> intrusion into the secondary containment. <br /> CORRECTIVE ACTION: Correct immediately by having a properly licensed,trained, and certified contractor replace the failed <br /> component with a functional component(LG 113-x listed, if applicable), and obtain a permit within one business day from the <br /> EHD, if needed. If the failed component can't be replaced immediately,there is a possibility that the UST system may be <br /> red tagged to prevent fuel inputs. <br /> This is a Class II violation. <br /> OVERALL NOTES AND COMMENTS <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the <br /> corrective actions that have been or will be taken for each violation, and any supporting paperwork, by 12/25/2020 <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate($152).
