Environmental Health Department <br />Underground Storage Tank Program Compliance Review Report <br />Facility Name and Address: Harney Lane 76, 141 E HARNEY LN, LODI 95240 <br /> #106 CCR 2632(d), 2641(h) Failure to submit and maintain a current facility plot plan / site map <br />OBSERVATIONS: OBSERVATION: An accurate UST Monitoring Site Plan was not submitted. <br />REGULATION GUIDANCE: A site plan must be submitted identifying the locations where monitoring will be performed. The <br />site plan must show the general layout of tanks and piping and clearly identify locations of the following equipment, if <br />installed: 1) monitoring system control panels; 2) in-tank liquid level probes (if used for leak detection); 3) devices monitoring <br />tank annular spaces or vault; 4) devices monitoring product piping; 5) devices monitoring fill piping; 6) devices monitoring vent <br />piping; 7) devices monitoring vapor recovery piping; 8) devices monitoring vent/transition sumps; 9) devices monitoring <br />under-dispenser containment; 10) line leak detectors; and 11) devices monitoring any other secondary containment areas. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. <br />This is a Class II violation. <br />#106 CCR 2632(d), 2641(h) Failure to submit and maintain a current facility plot plan / site map <br /> #203 CCR 2712(b)(1-2) Failure to maintain monitoring records for 36 months and maintenance records for 60 <br />months <br />OBSERVATIONS: OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. <br />--Alarm logs of the monitoring panel were not found. <br />-Alarm logs are not being maintained for the vacuum alarms on site. <br />REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records include: <br />(1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of <br />any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or <br />other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and <br />reconciliations. <br />CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last three <br />years on site and submit copies to the EHD. <br />This is a Class II violation. <br />#203 CCR 2712(b)(1-2) Failure to maintain monitoring records for 36 months and maintenance records for 60 months <br /> #205 CCR 2712(i) Failure to have current UST Monitoring Plan readily accessible at the facility <br />OBSERVATIONS: OBSERVATION: An approved copy of the monitoring plans were not available for inspection. <br />REGULATION GUIDANCE: A copy of these plans shall be accessible on site at all times. <br />CORRECTIVE ACTION: Immediately locate and retain a copy on site. <br />This is a Class II violation. <br />#205 CCR 2712(i) Failure to have current UST Monitoring Plan readily accessible at the facility <br />Rev. 9/16/2020 Underground Storage Tank Program Compliance Review Report <br />FA0024750 PR0543574 SC021 11/25/2020 <br />Page 2 of 4 <br />1868 E.
