Environmental Health Department <br />Underground Storage Tank Program Compliance Review Report <br />Facility Name and Address: Harney Lane 76, 141 E HARNEY LN, LODI 95240 <br /> #206 CCR 2712(i), 2634(e) Failure to have a current UST Response Plan available on site <br />OBSERVATIONS: OBSERVATION: An approved copy of the response plan was not available for inspection. <br />REGULATION GUIDANCE: A copy of this plan shall be accessible on site at all times. <br />CORRECTIVE ACTION: Immediately locate and retain a copy on site. <br />This is a Class II violation. <br />#206 CCR 2712(i), 2634(e) Failure to have a current UST Response Plan available on site <br /> #208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements <br />OBSERVATIONS: OBSERVATION: The designated operator failed to document all the alarms on designated operator <br />inspection report and failed to check that they were responded to appropriately. <br />REGULATION GUIDANCE: During the inspection, the designated operator shall review the alarm history since the previous <br />inspection, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history <br />with documentation taken in response to any alarms to the inspection report. <br />CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the <br />required information on the reports. <br />This is a Class II violation. <br />#208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements <br /> #210 CCR 2715(c) Failure to provide initial training, train every 12 months, and have a trained employee present <br />OBSERVATIONS: OBSERVATION: The designated operator employee training has not been completed. <br />REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and maintenance <br />of the UST system at least once every 12 months. The training shall be conducted before the individual performs the duties <br />of a facility employees. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility’s best management practices <br /> 2. Employee’s role with regard to monitoring equipment as specified in the facility’s monitoring plan <br /> 3. Employee’s role with regard to spills and overfills as specified in the facility’s response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Initial training shall also include a site-specific practical demonstration that provides both sharing information and evaluating <br />knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills.
