S A N J 0 A Q I I I Irl Environmental Health Department <br /> CCUN r <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> DICKS SPORTING GOODS#1297 2770 REYNOLDS RANCH PKWY, LODI August 31, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> OBSERVATION: At the time of inspection, it could not be demonstrated (not necessarily documented)that <br /> employees who handle hazardous waste were properly trained, based on the following observation: <br /> 1.At the time of inspection,Yazmine Trevizo,the employee who generally manages the hazardous waste storage <br /> area, according to Robert Leal,the assistant store manager, placed a 12 oz carbon dioxide cylinder into an <br /> Hazardous Waste Aerosols storage container without completing the hazardous waste label on the container.The <br /> label was missing the generator name and the accumulation start date. <br /> 2.According to a hazardous waste storage area weekly inspection checklist dated 6/9/20, completed by Yazmine <br /> Trevizo, and found in the hazardous waste storage area, a spray paint can and two carbon dioxide cylinders were <br /> disposed into the Hazardous Waste Aerosols container the week of 6/9/20. However, at the time of inspection, only <br /> one carbon dioxide cylinder was found in the hazardous waste storage area,which had been placed in the storage <br /> area on the date of inspection by Yazmin.Yazmin stated that Stericycle,the facility's hazardous waste hauler, had <br /> picked up spray paint can and the two carbone dioxide cylinders, but didn't leave the disposal records on-site. <br /> Robert Leal stated that the store usually has to request the disposal records from Stericycle on a date after the <br /> waste has already been picked up. <br /> 3.An All-Star Tiger Stick batting grip was found leaking into the item's own packaging, inside the Defectives storage <br /> container.According to Robert, defective items are sent to the facility's distribution center, located in Goodyear, <br /> Arizona. Robert indicated that the leaking batting grip could be managed as hazardous waste. Robert stated he <br /> would have to go into the facility's WERC computer system to determine which hazardous waste container the <br /> batting grip should be placed.At the time of inspection Robert was unable to login to the WERC system. <br /> REGULATION GUIDANCE: The business owner must ensure that all employees are thoroughly familiar with proper <br /> waste handling and emergency procedures, relevant to their responsibilities during normal facility operations and <br /> emergencies. <br /> CORRECTIVE ACTION: Immediately train or re-train the employees.
