SAN J 0 A Q I I I Irl Environmental Health Department <br /> CCUNT <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Party City#935 Lodi 2350 W Kettleman Ln, Lodi September 01, 2020 <br /> REGULATION GUIDANCE: Any person who generates a waste shall determine if the waste is a hazardous <br /> waste as specified in Title 22 California Code of Regulations (CCR). There are wastes that are listed as <br /> hazardous wastes. There are wastes that exhibit one or more of the hazardous waste characteristics: toxic, <br /> corrosive, reactive or ignitable. <br /> CORRECTIVE ACTION: Immediately secure the container of suspected used oil, make a hazardous waste <br /> determination for the material, and manage it according to Title 22 CCR. Use Safety Data Sheets (SDS), waste <br /> sampling and test results or other knowledge to support your hazardous waste determination. Waste testing <br /> must be done using methods specified in Title 22 CCR including sections 66261.20-24. <br /> Submit a statement and supporting documentation with your hazardous waste determination. Demonstrate <br /> current storage and labeling for wastes determined to be hazardous wastes. Provide disposal records for <br /> wastes determined to be hazardous wastes and stored longer than the number of days specified in Title 22 CCR <br /> for your hazardous waste generator status. <br /> 2. OBSERVATION: Hazardous waste containers were observed on-site with incomplete labeling. The <br /> containers were: <br /> 1. A 15-gallon hazardous waste drum storing Hi-float balloon treatment liquid, popped balloon containing <br /> Hi-float, and pens, was missing not marked 'liquid"for the physical state and was inaccurately labeled as storing <br /> cosmetics for the composition. <br /> 2. A 15-gallon hazardous waste drum storing silica gel packets, candles, and crayons, was inaccurately labeled <br /> as storing cosmetics for the composition. <br /> REGULATION GUIDANCE: All hazardous waste containers shall be marked with the following information: the <br /> words "Hazardous Waste", name and address of generator, hazardous properties, physical state, composition <br /> (contents), accumulation start date <br /> CORRECTIVE ACTION: The violation was corrected during the inspection by Nicole Valdez, the store <br /> manager, who corrected the hazardous waste labels. No further action is required to correct this violation. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 10/1/2020. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
