San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone: (209)468-3420 Fax; (209)468-3433 Web:www.s'gov.org/ehd <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> JD Service Station, Inc. 9015 WALNUT GROVE RD, THORNTON April 09, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> -A designated operator inspection was performed on 12/28/18 and the next inspection was performed on 1/31/19, <br /> 34 days later. <br /> -A designated operator inspection was performed on 2/23/19 and the next inspection was performed on 3/29/19, 34 <br /> days later. <br /> The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. <br /> Ensure that all designated operator inspections are performed within the required time frames. <br /> The designated operator failed to check and note all required items on the visual inspection reports. <br /> -The 10/30/18, 11/29/18, and 12/28/18 inspection reports are on outdated forms. <br /> -Tank tightness and line tightness test information was left blank on the 1/31/19, 2/23/19, and 3/29/19 reports. <br /> -CERS ID and zip code were not included on the 2/23/19 and 3/29/19 reports. <br /> -The 1/31/19, 2/23/19, and 3/29/19 reports left blank the question regarding UDC sensor locations. <br /> -The 1/31/19, 2/23/19, and 3/29/19 reports mark that sumps did not need to be inspected, but also state three <br /> sumps were opened and looked inside. <br /> -The question regarding sump sensor locations was left blank on the 1/31/19, 2/23/19, and 3/29/19 reports. <br /> -The section regarding fill pipe obstructions was left blank on the 1/31/19 report. <br /> -The 1/31/19, 2/23/19, and 3/29/19 reports contain incorrect overfill testing information. <br /> -Section V of the 1/31/19 and 3/29/19 reports was filled out by the designated operator instead of by the site <br /> owner/operator, and did not include corrective actions. <br /> -The 1/31/19, 2/23/19, and 3/29/19 reports do not contain a designated operator signature. <br /> -The items marked N and NA were not included in Section III on the 1/31/19, 2/23/19, and 3/29/19 reports. <br /> -The 2/23/19 and 3/29/19 reports do not contain the owner/operator signature. <br /> -The 2/23/19 and 3/29/19 reports incorrectly state all follow up actions have been corrected, but list the same issues <br /> as previous months. <br /> During the visual inspection, the designated operator shall check that all required testing and maintenance for the <br /> UST system have been completed and shall check all required items on the report. Ensure that designated <br /> operators performing visual inspections at this facility are including all of the required information on the reports. <br /> The designated operator failed to document all the alarms from the attached alarm history on the designated <br /> operator inspection report and failed to check that they were responded to appropriately. The missing alarms <br /> include: probe out alarms on 9/26/18 on all three tanks. During the visual inspection, the designated operator shall <br /> review the alarm history for the previous time period, check that each alarm was documented and responded to <br /> appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the <br /> inspection report.
