Current Financial responsibility documents have not been submitted to the EHD. Financial responsibility documents uploaded to CERS were last signed on January 29th, 2018. Current financial responsibility documents are required to be submitted annually. The operator was able to log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, and uploaded the required documents to correct this violation while the inspector was on site. This was corrected on site. This is a repeat violation, Class Il. 105 CCR 2632(d)(1), 2634(d) Failed to have an approved UST monitoring plan. The monitoring plan is not current and/or not approved by the EHD. The monitoring plan list TLS350 as the model of the panel, however Gilbarco, EMC panel was observed during the field inspection. Furthermore the leak sensor manufacturer and leak sensor model for the UDCs was not listed in the monitoring plan. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). The operator logged into CERS, and made the necessary changes, and submitted for review by the EHD while the inspector was on site. This was corrected on site. This is a repeat violation, Class II. 110 CCR 2637(e), 2637(f) Failed to submit"Secondary Containment Testing Report Form"within 30 days after the test. Secondary containment testing was performed on 7/19/18 and a test report was submitted on 3/29/19, over 7 months late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a Class II violation. 113 CCR 2638(c), 2638(d) Failure to submit"Monitoring System Certification Form"within 30 days of completion of the test. Annual monitoring system certification, and leak detector testing,was performed on 4/2/18 and a test report was submitted on 7/2/18, 3 months late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner.
