A copy of <br /> the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and <br /> submittal of test reports are performed in a timely manner. <br /> This is a Class II violation. <br /> 204 CCR 2712(1)Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be <br /> accessible on site at all times.The operator made the correction of to the monitoring plan and made a copy to keep <br /> on site while the inspector was on site to correct this violation. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> The designated operator failed to document the following: <br /> On the 10/30/18, 11/28/18, 12/27/18, 1/24/19, 2/21/19, 3/21/19 and 4/18/19 designated operator(DO)inspection <br /> reports under <br /> -Section IX <br /> The DO failed to mark Yes, No or N/A to the question"Are all sensors in under-dispenser containment located to <br /> detect a leak at the earliest opportunity?" <br /> During the designated operator inspection,the designated operator shall review the alarm history for the previous <br /> month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm <br /> history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators <br /> performing inspections at least once every 30 days at this facility and are including all of the required information on <br /> the reports. <br /> This is a repeat violation, Class II. <br /> 318 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gallons per hour leak at 10 pounds per square inch. <br /> The Diesel product line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of <br /> detecting a 3-gallon per hour leak at 10 psi.The service technician opened the Red Jacket FXIVD and found a piece <br /> of rubber trapped in the leak detector.
