Within 30 days, July 1, 2010, submit copies to the DTSC and provide this office with proof of submittal. Violation #60-Failed to keep signed TSDF copy of manifest for 3 years 66262.40(a) CCR. Violation #62-Failed to file a DTSC report 25123.3(h)(2) California Health and Safety Code (HSC). Hazardous waste generators must maintain copies of TSDF-signed manifests for 3 years and have them readily available for review. The generator of >1000kg/mo, who does not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility within 35 days of the date the waste was accepted by the initial transporter shall contact the transporter and/or the owner or operator of the facility to determine the status of the waste. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 60 days (45 days for generators >1000 kg/mo) from the date that the hazardous waste was accepted by the initial transporter, the generator shall submit to DTSC a legible copy of the manifest, with some indication that the generator has not received confirmation of delivery. At time of inspection, copies of consolidated manifests, for the previous 3 years could not be provided. Violation #79-Failed to store Universal Waste (UW) lamps in a closed container and in a manner that prevents breakage and release to the environment 66273.33(b) CCR: At time of inspection, several lamps, including 7 4-ft lamps, and 3 6-ft lamps, stored next to the lamp storage boxes in the electrical room, 2 lamps stored under the stairwell in the electrical room, and 1 lamp stored in the trash can in the building services cage, were seen improperly stored. Immediately, store all universal waste lamps pursuant to 66273.33(b) CCR for future storage. Violation # 83-Failed to label UW to identify type of UW 66273.34 CCR.
