HARNEY LN . , LODI , CA 95240 <br /> The San Joaquin County Environmental Health Department ( EHD ) has completed the review of the application <br /> for a permit to install a Xerxes new underground storage tank ( UST) system at the above noted address , <br /> which will consist of: <br /> TA0520849 20 , 000 gallon regular gasoline <br /> TA0520850 12 , 000 gallon diesel (split tank) <br /> TA0520851 8 , 000 gallon premium gasoline (split tank) <br /> The permit has been approved with the following conditions : <br /> 1 . Contact the local Fire District, the San Joaquin Valley Air Pollution Control District, and the local <br /> Community Development Department for applicable permits . <br /> 2 . Any changes shall be submitted to the EHD for approval prior to commencing work, including <br /> change in contractors , service technicians , installers , etc . All new submittals supersede the <br /> previous ones . <br /> 3 . Qualified installers must possess or work under the direct and personal supervision of an individual <br /> physically present at the work site , at all times while work is being performed , who possesses all of <br /> the following : <br /> • One of several permissible valid , current contractor' s licenses issued by the Contractors State <br /> License Board ( may be issued to the installation company) , <br /> • A valid , current UST Installation/ Retrofitting certificate issued by the International Code Council <br /> ( ICC ) , <br /> • A valid , current certificate of training from the manufacturer( s ) of the component( s) being <br /> installed . <br /> 4 .
