The designated operator failed to document one or more of the following: On the 11/28/18, 12/31/18, 1/29/19, 2/28/19, 3/29/19, and 4/29/19, designated operator(DO) inspection report under -Section III The DO failed to add a description to all the components marked N/A or N from section VII through XI. -Section V The owner/operator failed to provide explanation of follow-up actions that will be taken for the statement provided by the DO in section III. -Section VII The DO fail to mark yes or No for the question "Has each follow-up action of section III from the previous inspection been completed appropriately?" -Section VIII The designated operator failed to attached documentations as required by this section. The DO failed to mar YES, NO or N/A under the question "Has the tank tightness testing been completed within required timeframes?" and "Has the line tightness testing been completed within the required timeframes?" -Section IX The DO failed to check if all containment sumps that have had a leak detection alarm since the previous inspection and have not been responded to by a qualified UST service technician. -Section X The DO failed to mar YES, NO or N/A under the question "Has the overfill prevention equipment inspection been completed within the past 36 months?" Furthermore: The DO did not conduct an inspection within 30 days of the previous inspection. The DO inspected the facility on 10/26/18 and on 11/28/18. The DO report was conducted 33 days after the 10/31/18 inspection and not within 30 days as stated by regulation. The DO did not conduct an inspection within 30 days of the previous inspection. The DO inspected the facility on 11/28/18 and on 12/31/18. The DO report was conducted 33 days after the 11/28/18 inspection and not within 30 days as stated by regulation. The DO did not conduct an inspection within 30 days of the previous inspection. The DO inspected the facility on 3/29/19 and on 4/29/19. The DO report was conducted 31 days after the 3/29/19 inspection and not within 30 days as stated by regulation. During the designated operator inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing inspections at least once every 30 days at this facility and are including all of the required information on the reports.
